    WE, HERMINIGILDO H. BLANCO and FELINA G. BLANCO both of legal age,


    Filipino, with residence and postal address at No.103 A. Tingluyan Capipisa, Tanza,
    Cavite, after having been duly sworn to in accordance with law, hereby depose and say
    that:

    We are the parents of ARIES BLANCO;

    We are hereby giving consent to the marriage of our son;

    This consent is declared to the authorized Civil Registrar for their application for
    marriage license as requirements for their marriage and to the Solemnizing Officer that
    is in charge of this matter;

    We are aware of the fact that our declaration of consent becomes irrevocable
    once it has been received by the Civil Registrar and Solemnizing Officer.

    has been received by the guardianship in court.
