    AFFIDAVIT OF LOSS

    I, JOSELITO M. JOSE, of legal age, Filipino, single, and a resident of


    Sampaloc Manila, Philippines, after being duly sworn according to law, hereby
    depose and state that:

    I am a holder of Post Office Postal Identification card with issued by the


    Post Office of Manila;

    Sometime in 3 January 2019, while at the town proper, I lost my wallet and
    inside is the above-mentioned ID;

    Despite diligent efforts exerted, I could no longer locate, find, recover the
    said identification card, and to the best of my knowledge, the same is truly lost;

    The said Postal ID has not been confiscated by any officer of the law, of the
    issuing office or agency due to violation of any law, statute, order rule or
    regulation;

    I am executing this Affidavit to attest to the truth of the foregoing


    statement and to request the issuance of new Postal ID in lieu of the one that was
    lost, or for whatever legal purpose this may serve.

    IN WITNESS WHEREOF, I have hereunto set my hands this 31st day of


    January 2019 at Zamboanga City.
