Students are requested to complete this questionnaire, as part of the UFS process of
assuring the quality of academic modules. Your honest response is highly valued, and
will be treated with strict confidentiality. Please take care to answer the following
questions thoughtfully, accurately and fairly. Give your own opinion, indicating whether
you agree or disagree with each statement. Use the following key, and circle the item on
the rating scale that corresponds most closely to your experience:
