MEMORANDUM OF AGREEMENT

This Memorandum of Agreement is entered into this ____ day of SEPTEMBER 2019 in Hanawan, Ocampo, Camarines Sur, Philippines, by and
between:

SHEENA C. RUFO, Filipino, of legal age, a duly appointed teacher of the SENIOR HIGH SCHOOL DEPARTMENT of HANAWAN
NATIONAL HIGH SCHOOL under Department of Education, Camarines Sur Division, with School Identification Number 309704 and office
address Hanawan, Ocampo, Camarines Sur, and hereinafter referred to as the TEACHER;

-and

JOHN MAR D. DELOS SANTOS (Student), of 12 GAS (Grade and Section), a resident of _____________________________________, and
represented in this Agreement by) JESUS B. DELOS SANTOS (parent/guardian, Filipino, of legal age, hereinafter referred to as the
Parent/Guardian and STUDENT.

WITNESSETH:

WHEREAS, the Department of Education of the Philippines, hereinafter referred to as “DepEd”, is the primary government instrumentality
mandated to formulate, implement, and coordinate objectives, standards, policies and programs in the areas of formal and non-formal basic
education; supervise all elementary and secondary education institutions, including alternative learning systems, both public and private; and provide
for the establishment and maintenance of a complete, adequate, and quality integrated system of basic education relevant to the goals of national
development;

WHEREAS, the DepEd has introduce the K-12 basic education reform program that includes Senior High School, hereinafter referred to as “SHS”,
with the major objective of ensuring that graduates of basic education are competent, ready for employment, entrepreneurship and higher learning;

WHEREAS, the SHS curriculum take into consideration the learning needs of the students, set learning standards for quality of education and the
means and ways of attaining the learning objectives;

WHEREAS, the school is among those that will offer SHS to students in the community to carry out DepEd’s objectives for SHS, in accordance to
DepEd’s vision, mission, goals, objectives, policies, programs and set education-learning standards;

WHEREAS, to achieve quality education and learning, the school shall exercise its duly given authority in full effort and optimism;

WHEREAS, the TEACHER shall perform, by persistent means and untiring effort, its duties and responsibilities accordingly considering DepEd’s
vision, mission, goals, objectives, policies, programs and set education-learning standards;

WHEREAS, the TEACHER, after exhausting means and effort, has accordingly and constantly reminded the student of its performance in the
subject PERSONAL DEVELOPMENT, taken this 1ST Semester of Academic School Year 2019-2020

WHEREAS, the STUDENT considers its part in fulfilling this objective;

WHEREAS, the TEACHER and the STUDENT, hereinafter collectively referred to as “parties”, undertake to collaborate for the successful
implementation of the course curriculum in SHS in cognizant of the needs of the child with the best interest of the SHS learner at heart;

NOW, THEREFORE, for and in consideration of the foregoing premises, the parties hereby agree as follows:

Section 1. RESPONSIBILITIES OF THE PARTIES

The TEACHER shall:


a. Assess student’s learning needs
b. Plan objectives and strategies based on student’s learning needs considering DepEd’s quality education standards
c. Design and implement learning-need-appropriate activities
d. Provide student with compensatory activities as applicable
e. Ensure that the student will adhere to the policies, plans, programs and standards of Hanawan National High School (Senior High
School Department) and DepEd
f. Monitor student’s progress
g. Provide student, parents/guardian with appropriate feedback and update
h. Issue a FINAL PASSING GRADE to the student only upon completion of the course requirements, obtaining at least
75% as equivalent grade
The STUDENT shall:
a. Satisfactorily meet the terms and requirements of the course, having at least 75% equivalent grade
b. Comply the given compensatory activities within the prescribed period as applicable
c. Consistently abide to the policies, programs and standards of Hanawan National High School (Senior High School Department)
and DepEd

Section 2. EFFECTIVITY

This agreement shall take effect upon signed until amended, terminated, or requirements have been completed.
