13. Enumerate appropriate discharge plan and health teaching for your patient.

DISCHARGE PLAN

MEDICATION

On the 6th day, the physician ordered may go home with the following instructions:
Continue medication of:
 Digoxin 0.25 microgram/ tab once a day
 Furosemide 40 mg/ tab twice a day
 Metformin 500 mg/ tab twice a day
 Captopril 25 mg/ tab once a day
 Advise the patient to report early signs of digitalis toxicity such as anorexia, nausea and
vomiting, and visual disturbances
 Inform the patient of the signs and symptoms of adverse drug effect.
 Instructed patient not to take any medication without prescription
 Instructed to continue taking the prescribed medications.
 Home medications at right time, dose, route as ordered
 Encouraged to take the take home meds religiously

ENVIRONMENT
 Instructed patient to avoid extremes in heat and cold which increase the work of the
heart
 Encourage for humid environment
 Visitors be permitted but to sit quietly with short period of time
 Instructed patient to clean their surroundings
 Encouraged to practice proper hand washing
TREATMENT

 Advised to notify the nurse or physician on arrangement of follow up check ups


 Encouraged to seek regular consultations for monitoring of health condition
 Advise patient to look at all labels to ascertain sodium content such as laxatives and
antacids
 Follow up one week after discharge with the laboratory exam results
 Advised to practice proper hygiene
Health Teaching

 Teach the patient to take and record his pulse rate and blood pressure
 Provide written instructions concerning pharmacologic therapy
 Instructed patient to record her weigh
 Teach the patient the importance of low sodium , low fat diet
OBSREVATION OF SIGNS AND SYMTOMS

 Teach the patient the signs and symptoms of recurrence such as:
• gain weight,
• swelling of ankles,
• swelling feet and abdomen
• Persistent cough
• Fatigability
• Frequent urination at night

DIET AND EXERCISE

 Advised patient increase walking gradually, provided they do not cause fatigue and
dyspnea
 Instructed patient minimal walking with assistance, avoid going up and down the stairs
 Advised patient to avoid excessive eating and drinking
Diet on a daily basis to maintain a healthy lifestyle
• Encourage to eat low salt, low fat with banana
 Encourage to drink 2000 to 3000mL of fluid daily to
Facilitate defecation
 Encouraged to take meals on time.
 Advised to have optimum nutrition including vitamins
SPIRITUALITY

 Advised patient to visit church


 Advised patient to pray before bed time
 Advised to always faith in God to strengthen spiritual relationship with the latter
