Introduction

The patient is a 35-year-old male who has a three-month-old newborn baby he

currently stays at home with during the week while his wife works. He is overweight and

has family history with heart disease. A thorough health history and assessment were

conducted where the patient reports difficulty sleeping, and a desire to lose weight.

Additionally, he reports significant stress related to his newborn baby and work

environment. His health history assessment revealed a family history of diabetes,

hypertension, heart disease and obesity. After a thorough assessment of the patient was

completed, a plan of care was developed to address concerns and promote prevention of

disease processes revealed in his family history.

Assessment & Plan of Care

Weight

Patient reports having an eating addiction and struggles with his weight. Patient

has been overweight most of his life and states his family members have struggled with

obesity. During the assessment the patient points to childhood factors that caused weight

gain, specifically family eating dynamics and his inability to control portions. Some

suggested remedies include measuring meals, making more nutritious choices, and using

internet resources to assist with planning his diet. It was also suggested that he join a

gym or participate in a sport. During the assessment, the client expressed a desire to join

a martial arts gym and use a phone app to help him manage meals. The patient was

provided internet resources of martial art gyms, sports teams in Delaware and phone apps

that track meals and food intake.

Risk for Heart Disease

The patient states he lost both grandfathers to heart disease and his father

currently suffers from diabetes. The patient is overweight and wishes to address his

weight with his risk in developing heart disease and/or diabetes. Further, the patient feels

he is addicted to eating and will have a difficult time changing his behaviors. Though, the

patient is willing to make changes as he now has a baby boy and wants to live longer.

The patient currently has a healthy attitude and motivation toward changing his diet and

exercising. He was provided with information pertaining to hypertension and encouraged

to continue follow ups with his physician to monitor his blood pressure and maintain age

appropriate screenings.

Conclusion

The patient seems to want to make positive changes towards losing weight and

reducing his risk for heart disease. He displays a willingness to use the information

provided to make life changes. With the lack of sleep and more stress involved with a

new baby, a healthy behavior to mediate his risk for heart disease is crucial. Eating

healthy and less can also reduce the risk of diabetes. The patient understands his current

and potential health complications and is motivated to change his lifestyle. The patient

appreciated this assessment and will follow the plan of care as close as possible.
