    AFFIDAVIT OF DENIAL

    I, JANZEL KERCH BARRERA GUCOR, of legal age, Filipino, single, and a


             
    resident of Bontores St., So. Villa Kalubihan, Brgy. Basak San Nicolas, Cebu City,
    Cebu, Philippines, after being sworn to in accordance with law, depose and state -
    THAT:

    1.  I am applying for a Globe Postpaid plan with Globe Communication, Inc.

    2. To comply with this requirement I submitted my application thru online and


    after verification;

    3. The Customer Service Representative found out in their system that I have
    an unpaid account with the two (2) account number stated in the table
    below;

    AMOUNT PHP. 7,197.43 AMOUNT PHP. 587.50

    4. I am executing this affidavit to attest to the foregoing facts and to deny


    categorically and specifically under oath that I am not the person applied
    nor used the said account;

    5. I was born on May 24, 1995 in Silay City Negros Occidental, and I have
    established my permanent residence in Bidan St., Zone I, Brgy. II, Manapla
    Negros Occidental;

    6. In view of the foregoing, I am requesting the Globe Communication Inc.


    Department incharge to validate, investigate and clear my name in the
    system so that I may proceed with my application;

    IN WITNESS WHEREOF, I hereby affixed my signature this 3rd day of February


    2020 in Cebu City, Cebu, Philippines.
