On or about Feburary 12, 2021 Galen Phillips did knowingly and intentionally, during the course of representing Zello, Inc. in
Fulkerson v. Zello, Inc. A Texas Corporation, American Arbitraton Association Case No. 01-20-0010-1019, submit falsified
information upon the record with the intent to mislead the tribunal, to-wit a statement that Zello Inc.'s web app contained a link
to a terms of service contract, citing a screenshot as evidence, but maliciously left out information indicating that his client had
updated the web application on December 30, 2020. A motion to strike the offending documents was promptly filed upon the
record in the original proceedings. Previously to this a complaint against Mr. Phillips was filed with both the Oklahoma Bar and
the California Bar on grounds that Mr. Phillips engaged in the unlicensed practice of law in the state of Oklahoma. The
Oklahoma Bar Complaint is still under active investigation to this reporters knowledge. Supporting documentation is attached.
It should be noted that this reporter is a disabled adult pursuant to ruling under the authority of the United States Social
Security Administration Administrative Law Judge. There is nothing further to report at this time.
