FLUIDTHERAPY

STEP 1: PERFUSION
If >40ml/kg dog; 30ml/kg cat CSx of Perfusion Deficits: End Point Resuscitation Variables:
WHAT: with no clinical improvement • Poor mentation
Correction of consider: • Reduced pulse amplitude VARIABLES: VALUE:
intravascular fluid • Cardiogenic shock: Jugular • Increased heart rate
deficit. Mentation Alert
pulses, heart murmur, • Pale mucous membranes
arrhythmia • Prolonged capillary HR (bpm) Dog: 90-140
HOW: • Obstructive shock: GDV, refill time Cat: 180
• 10ml/kg crystalloid pericardial effusion, tension • Reduced core body MM Pink
boluses, repeated while pneumothorax temperature CRT 1-2
reassessing clinical • Distributive shock:
Rectal Temp (°C) >37.5
signs of perfusion Anaphylaxis - trial
deficits and vasopressor therapy MAP (mmHg) 70-80
end-point resuscitation • Ongoing losses: Internal Systolic BP (mmHg) 100
variables. haemorrhage - consider Sp0 2 (%) >95
• Head trauma? Consider blood transfusion
Lactate (mmol/L) <2
Hypertonic saline 7%
at 3ml/kg slow IV. Base deficit (mEq/L) > -4

IV FLUID RATE = HYDRATION DEFICIT + MAINTENANCE REQ. + ONGOING LOSSES

STEP 2: REHYDRATION FLUID DEFICIT (L) = % DEHYDRATION X BWT (KG) / 100

WHAT:
Correction of interstitial fluid deficit. Step 2: Select degree of dehydration

HOW:
Correct 50% of the fluid deficit over the first 6 hours
then the remaining 50% over the next 18 hours.

FLUID CALCULATION

STEP 3:
CHART

MAINTENANCE
WHAT:
Daily requirement to
meet metabolic needs.
HOW:
In addition to rehydration rates, based on:
Cats: 80 x BWT 0.75 = ml/day
Dogs: 132 x BWT 0.75 = ml/day
CANINE

STEP 4:
FLUID CALCULATION CHART

ONGOING LOSSES
WHAT:
Replacement of ongoing losses, for
example vomiting, diarrhoea, polyuria.
HOW:
Estimate or weigh losses
(1gm of liquid = 1ml of fluid)

EXAMPLE:
5kg cat, 5% dehydrated, 100mls diarrhoea per 24hrs
1. Correct perfusion deficit eg. 10ml/kg bolus
2. Hydration req for 5% dehydration = 250ml/24hrs
- First 6 hours = 21ml/hr
- Next 18 hours = 7ml/hr
3. Maintenance req for 5kg cat = 267ml/24hrs
- Rate per hour = 11ml/hr
4. Ongoing losses for 100ml of diarrhoea = 100ml/24hrs
- Rate per hour = 4ml/hr
Overall IV fluid rate:
- First 6 hours = 21 + 11 + 4 = 36ml/hr
- Next 18 hours = 7 + 11 + 4 = 22ml/hr
Step 3: Add maintenance requirement
