    AFFIDAVIT OF SELF-ADJUDICATION

    REPUBLIC OF THE PHILIPPINES)

    CITY OF MANILA ) S.S.

    I, LEONORA JOAQUIN, of legal age, Filipino, and presently residing at 1281 Alfonso Mendoza
    St., Sampaloc, Manila, after having been duly sworn to in accordance with law, depose and
    state that:

    1. I am the sole heir of the late Ignacio Joaquin and Amparo Joaquin, being their only
    biological daughter;

    2. Both Ignacio and Amparo passed away on 10 April 2021 without any last will and
    testament;

    3. At the time of their deaths, they had no debts, liabilities or obligation to pay to any persons,
    agency or institution;

    4. The properties left by my parents are as follows:

    A. Insurance policies amounting Php100,000.00 each or a total of Php200,000.00;

    B. Monies in banks in the total amount of Php150,000,00;

    C. The wrecked vehicle with depreciated value of Php25,000.00; and

    D. House and lot valued at P350,000.00, the lot more particularly described as follows:

    A PARCEL OF LAND (Lot 32 of the consolidation-subdivision plan (LRC)


    Pcs-5141, being a portion of the consolidation of Lots 1 and 2,
    Psu-112287 Amd., LRC (GLRO) Rec. No. N-17511), situated in the Dist. of
    Concepcion, City of Sta. Rosa, Laguna, Island of Luzon. Bounded on the
    NE., points 3 to 6, by Lot 85; on the S., points 6 to 1 by Lot 30; on the SW.,
    points 1 to 2, by Lot 31; and on the N., points 2 to 3 by Lot 35, all of the
    consolidation-subdivision plan. Beginning at a point marked “1” on plan,
    being N. 51 deg. 36’ E., 1321.76 m. From B.L.L.M. No. 1, Sta. Rosa,
    Laguna.

    5. As sole heir of herein decedents, I hereby adjudicate the abovementioned properties solely
    for and in my name as well as any and all liabilities arising from said property; and

    6. I hereby execute this a davit for the purpose of processing the transfer of the said property
    in my name, the settlement of the estates of the late Ignacio and Amparo Joaquin, as well as
    for the release of any other claim or bene t in relation to their deaths before any
    government/private o ce and banking institution and for any other legal purpose this may
    best serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 23rd of April 2021 at Manila,
