This survey is conducted, in response to the fatal occupational accident occurred at the Yonago Plant, Oji Paper Co., Ltd., by the fall and crash caused by the rupture of maintenance hatch while a driver of the quicklime tanker, a contractor who entered the facility temporarily, was working on top of the tank, at the direction of CEO Kaku, to investigate safety measures (to prevent accidents like ruptures and falls) during cargo handling (receiving and shipping, carrying in and out) on raw materials, fuels, products, and waste to and from tankers, trucks and dump trucks, and to prevent similar accidents by taking necessary measures.
<Accident summary>
This is what has been confirmed by Yonago Mill as of this moment It is hypothesized that the victim was thrown from the top part of the tank when unloading cargo after delivering quicklime to the Yonago Mill by lorry. There was a big noise at that time. The victim fell from a height of 3.5 m and hit his head on the ground.
A possible primary cause of the accident is the rupture of the tank with the resulting fall being a secondary cause. The rupture seems to have been caused by increased internal pressure, which caused the lid of the maintenance hatch on top of the tank on the lorry loading platform to blow out. This rupture caused the victim to fall 3.5 m from the top of the tank to the ground. (Skull fracture due to head bruise (cerebral contusion)
