1. What is the definition of Primary Health Care according to WHO? 
Primary Health Care [PHC], defined by the World Health Organization, primary health care is essential
health care based on practical, scientifically sound and socially acceptable methods and technology,
made universally accessible to individuals and families in the community through their full
participation, and at a cost that the community can afford to maintain at every stage of their
development in the spirit of self-reliance and self-determination. 
2. What is the strategic aimed of Primary Health Care? 
The  strategic aimed of primary health care   is  provide  essential health care that is Community- based;
Accessible; Part and parcel of the socioeconomic development effort of the nation; Acceptable;
Sustainable at an affordable cost. 
 3.Identify the importance of Health Promotion? 
Health promotion programs are designed to involve and empower individuals and communities to
make healthy choices, and to modify them to reduce the risk of developing chronic diseases and other
diseases.

1. Give the 3 levels of Health Prevention, describe and give each example? 
 Primary Prevention- Immunization against infectious disease is an example.  
 Secondary Prevention- Breast self-examination is an example.  
 Tertiary Prevention-example is regular physical activity which improve your health
and help prevent chronic diseases. 
 
 
 
2. Do nurses engaged in practice, do so in accordance with the beliefs of primary care and health
promotion? 
3. Discuss the Universal Health Coverage and the Phil. UHC signed by Pres. Rodrigo Duterte. 
 The Universal Health Care Bill, which led to massive reforms in the Philippines health care sector, was
signed in February 2019 by President Rodrigo Duterte. With UHC, all Philippines have fair access to
and protection against financial risk for quality and affordable health care goods & services.

Activity 2.1  The Primary Health Care aims to strengthen the health care system in the Phil., As a
Community Health Nurse, design strategies/interventions to increase opportunities and conditions,
wherein people will manage their own health care. 
 
Promoting breastfeeding- The best nutrition for children is breast milk. It has an almost perfect
vitamin, protein and fat mix – all your baby needs to grow. Everything is supplied in a way that is
easier to digest than infant formula. 
Promoting child and family nutrition- Because childhood is a time of rapid growth, development, and
activity, good nutrition is essential. This is also a critical period for tooth development and decay
prevention. In the first several years of life, general feeding habits and patterns are developed. 
Promoting physical activity- Muscles and bones are strengthened by regular exercise and physical
activity. It boosts your respiratory, cardiovascular, and general wellness. Maintaining a healthy
weight, lowering your risk of type 2 diabetes, heart disease, and various malignancies can all be aided
by staying active.

1. Why is family important in health care?  
A better relationship between healthcare professionals and family members benefits all parties
involved: healthcare professionals can benefit from family members' knowledge, family members
are better equipped to perform their care tasks, and patients receive better care because formal
and informal care are more aligned. 
 
 
2. What are some effects of family life on health?

3. What are the roles and responsibilities of a community health nurse?  
 
Roles and Responsibilities of Community Health Nurse are the following: 
 
1. Clinical Role- A community health nurse's clinician role entails providing health care not only to
individuals and families, but also to groups and populations in the community. 
2. Educator- Evaluates community members' knowledge, attitudes, values, beliefs, behaviors,
practices, change stage, and abilities, and delivers health education based on their knowledge
level. 
3. Advocate Role-A community health nurse acts as an advocate for a patient's right to care. They
encourage people to consume the correct foods to stay healthy, take the right drugs for
treatment, and get the right services at the right time, wherever they are required.  
4. Manager- As a manager the nurse exercises administrative direction towards the
accomplishment of specified goals by assessing clients’ needs, planning and organizing to meet
those needs, directing and controlling and evaluating the progress to assure that goal   is  met. 
5. Collaborator-Community health nurses seldom practice in isolation. They must work with many
people including clients, other nurses, physicians, social workers, and community leaders,
therapists, nutritionists, occupational therapists, psychologists, epidemiologists, biostatisticians,
legislators, etc. as a member of the health team (Fairly 1993, Williams, 1986). 
6. Leader- As a leader, the nurse instructs influences or persuades others to effect change that will
positively affect people’s health.  
7. Researcher- In the researcher role community health nurses engage in systematic investigation,
collection, and analysis of data to solve problems and enhance community health nursing

1. Discuss the Characteristics of a Healthy Family. Give example.

2. Explain the Health Tasks of the Family? 
 
*Physical maintenance- a family must provide its members with food, shelter, clothing, and
health care. 
*Family Socialization– The family must prepare children to live in the community and interact
with people outside the family. 
*Resource Allocation- the family must decide which family needs will be met and in what order. 
*Maintenance of Order– the family task entails establishing an effective means of
communication among family members, integrating family values, and enforcing common rules
for all family members. 
*Division of Labor – who will fulfill certain roles e.g., family provider, home manager, children’s
caregiver 
*Reproduction, Recruitment, and Release of family member 
*Placement of members into larger society –consists of selecting community activities such as
church, school, politics that correlate with the family beliefs and values 
*Maintenance of motivation and morale– created when members serve as support people to

3. Characterize the Stages of Family Development? 
 
 MARRIAGE & THE FAMILY 
 -Involves merging of values brought into the relationship from the families
of  orientation.Includes  adjustments to each other’s routines (sleeping, eating, chores,
etc.), sexual and economic aspects. 
EARLY CHILDBEARING FAMILY 
 -Birth or adoption of a first child which requires economic and social role changes  
 Oldest child: 2-1/2 years 
FAMILY WITH PRE-SCHOOL CHILDREN  
 -This is a busy family because children at this stage demand a great deal of time
related to growth and development needs and safety considerations. 
 Oldest child: 2-1/2 to 6 years old 
FAMILY WITH SCHOOL AGE CHILDREN  
 -Parents at this stage have important responsibility of preparing their children to be
able to function in a complex world while at the same time maintaining their own
satisfying marriage relationship. 
 Oldest child: 6-12 years old 
FAMILY WITH ADOLESCENT CHILDREN  
 -A family allows the adolescents more freedom and prepare them for their own life as
technology advances-gap between generations increases

1. Differentiate Family Assessment from Family Health Assessment?  
 
Family health assessment is a process by which information on health promotion and disease
prevention activities is received from the family. Family assessments include childcare insights
into the constitution, standards, standards, theoretical knowledge and abilities to communicate. 
 
2. Identify the difference of Health threats, Health deficits, Foreseeable 
Crisis?  
HEALTH DEFICIT is a disparity between one's expected and actual health status. Illness states,
whether diagnosed or undiagnosed, and failure to thrive/develop are examples (malnutrition) 
HEALTH THREAT- is a condition that makes one susceptible to disease, accident, or failure to
realize one's full  potential.Examples  include a family history of hereditary disease, the risk of

conditions.  
The process of determining existing and potential health conditions or problems in the family is
known as First Level Assessment. It describes what health problems exist and will exist in the
future.

Observing, interviewing, and examining are the primary data collection methods. Observation
occurs whenever the nurse has contact with the client or support personnel. Interviewing is
mostly used when taking a nursing health history. In physical health assessments, the most
