SYNOPSIS
1. TITLE
Smart Medical Card
2. INTRODUCTION
Our project " Smart Medical Card " is a project in which we are planning to help people in
managing their health information and making the process of carrying all previous medical
documents and prescriptions while visiting a doctor easier by the use of RFID Card and IOT.
By Using this Smart Card doctors can anytime see the medical details of the card holder like
what were the previous reports of the person and what were the medicines which were
prescribed to that person.
2.1 PROJECT BENEFITS

i) The amount of the people who will use it will increase as it’s very easy to take a card
rather than having n number of documents with them.
ii) This project is beneficial for the doctors as well as the medical staff as during the time
of this pandemic where everyone think many times before touching anything so we are
making their work easier as after this card, they don’t have to touch all the documents,
other than that they just have to scan the card and all the medical information, medical
history and prescriptions can be accessed. This will reduce the Medical staff’s risk as
well.
2.2 PROJECT SCOPE
The scopes of this project are:

 Provide the service to as many peoples as it will reduce their tension as well because
they don’t have to carry anything else other than that RFID Card.
 Work of Doctors as well as the medical staff can also be made easier as now they just
have to scan the card and the whole medical information of the patient can be seen
including the medical reports, prescriptions.
 This project can be considered most useful for the persons who have short term memory
loss as all the details of the patients is stored in that card. So, they don’t need to take
tension regarding that.

PROBLEM STATEMENT
We found this problem when we were visiting one of our friend as he was admitted in a
hospital. While waiting for him, we saw that an old lady was struggling with the medical
reports as her husband was admitted and there was no one to look after her and she wasn’t
able to understand that as she was in panic. We helped her over there and after that while
waiting, we got an idea that why not make a project for overcoming that problem as that
might be considered useful in future as the user can go hassle free for documents and
prescriptions as that data is stored on the server and what they have to carry is a card. Project
might be considered useful as it’ll also reduce the risk of medical staff.
4. OBJECTIVE
i) The main objective of this project is that people can go to consult any doctor hassle free
as there’s no need of taking the bag of previous medical reports and prescriptions as the
card contains all that details regarding that.
ii) Risk of Doctors can be lowered for looking for various documents and asking patients
again and again for every document as now doctor can see every document and
prescriptions on a single click.
5. INTENDED USER
The intended users are the peoples who are unable to keep their old reports and prescriptions
with them because they know that keeping them is important but they aren’t able to keep it. So,
for them this project might be considered useful.
6. EXISTING SYSTEM
In present scenario, there’s no such facility like the facility which we are proposing to give
where we can upload our medical reports and prescriptions and also there’s a risk for doctor
and the medical staff as they will have to touch each report and prescription as this is the time
of pandemic, where people think many times before touching anything despite of that risk,

Doctors are always ready. This project will reduce the risk of Doctors as well as their
medical staff. And also, there will be an ease in having a look on medical health from Doctor.
7. PROPOSED SYSTEM
The proposed system is “Smart Medical Card”. Here we are proving a card with RFID
which contains the data stored on the server and the data of that only patient can be
accessed by scanning that RFID Card. What we are doing is that we are giving access to
upload medical reports and prescriptions to the users so that they can put it on the server
and it can be called anytime when the hospital staff scans the card the details of that
particular patient can be seen on a single click.
7.3 HARDWARE REQUIREMENT
i) Internet
ii) RFID Device and Reader
iii) Arduino
8. EXPECTED OUTCOMES
i) The patient would be hassle free in the matter of taking reports and prescription with
him/her.
ii) A good step to go towards digitalization.
iii) Will reduce the tension among Doctors.
9. CONCLUSION
This is to conclude that the project that we undertook was worked upon with a sincere effort.
Most of the requirements have been fulfilled up to the mark and the requirements which
have been remaining, can be completed with a short extension. This project will satisfy the
requirements of the people as well as Doctors so that all of them will be hassle free.
9.2 FUTURE ENHANCEMENTS

Can be an Enhanced Digital Version of Govt. Provided Ayushman Card.
