    JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

    We, JONAH DURANGO AYOP and ROSALIA CA JOLES JARANTILLA, Filipino, of


    legal age, and residents of Tagbilaran City, Bohol, Philippines, after having been duly sworn to
    in accordance with law, do hereby depose and say:

    1. That we personally know ARNOLD PAYOT ARTABA, Filipino, 18 years old, single,
    and a resident of Tabalong, Dauis, Bohol, he being the son of a family friend;

    2. That his school records, with songculan Elementary School as per Elementary
    School Permanent Record, was enrolled therein as “ARNOLD PAYOT ARTABA”
    but in his Birth Certificate, he was registered as “ARNOLD PAYOT” issued by the
    City Civil Registrar;

    3. That we hereby attest that the name ARNOLD PAYOT ARTABA as appearing in the
    School Permanent Record and the name ARNOLD PAYOT in the abovementioned
    Birth Certificate refers to one and the same person, and that no other person shall
    answer for such name and circumstances except ARNOLD PAYOT;

    4. We are executing this Joint Affidavit to attest to the truth of the foregoing facts and
    for whatever legal purpose this may serve;

    IN WITNESS WHEREOF, we have hereunto set our hands this 15th day of JUNE 2021
    at the City of Tagbilaran, Bohol, Philippines.
