• PPO Comprehensive: Regular exams at 100%, basic services at 60%, major services at 50%

Vision Vision Care Coverage through Vision Service Plan:

Securing Employer Automatic Contributions: Employer-paid retirement benefit based on earnings and years of service
your Voluntary Employee Retirement Savings Plan Contributions: 403(b) retirement account that allows you to contribute on pre-tax
retirement or Roth (after-tax) basis; contribute as little as $1 or 1% up to 80% of earnings; annual associate contribution limits apply
Employer Matching Contributions to the Retirement Savings Plan*: Employer match (equal to 50% of first 6% of earnings you
contribute) if you make pre-tax contributions and meet requirements
*You will be eligible for the match if you are regularly scheduled to work 40 hours per pay period.
