    DRIVER’S AFFIDAVIT
    I, JOHN LESTER D. SABAYANAN, Filipino, of legal age, married, and a resident of Poblacion
    5 Cabadbaran, Agusan del Norte, Philippines, after being sworn to in accordance with law,
    DEPOSE and STATE:

    1. That I am a holder of a valid Driver’s License with No. K02-13-000468, and expiration
    date, 2024/ 04/07.

    2. That I drive a Motor Vehicle (MV) registered in the name of my wife, Ana Liezel B.
    Sabayanan, and which is more particularly described as follows:

    3. That on September 21, 2021, at about 7:29 in the evening, while our car was properly
    parked just outside our house, another car which happened to pass by, accidentally hit
    our car. As a result, it caused a dent or damage at our car’s body just above the right rear
    wheel, then slightly scratched towards the right rear door.

    4. The car that accidentally hit our vehicle is driven our neighbor, Virgin P. Cabana, likewise
    of legal age, Filipino, with residence at Blk 5, Lot 59 Lumina Homes, Taguibo, Butuan
    City.

    5. That such incident transpired despite exercising due diligence on our part.

    6. That I am executing this Affidavit to attest to the truth of the forgoing and in support of the
    claim against the insurer for the repair of the damage of the aforesaid vehicle and for
    whatever legal purposes.

    IN WITNESS WHEREOF, I have hereunto set my hand this 29th of September 2021 in
    Butuan City, Agusan del Norte, Philippines.
