    observed:

    1. There were no visible safety signage and early warning signs installed on the project
    site.
    2. Construction workers were not wearing proper Personal Protective Equipment.
    3. Provide Mixing pan During concrete mixing.

    In this regard, you are hereby instructed to make necessary actions regarding to above
    stated deficiencies.

    Please be informed that during site inspection last April 18, 2017 the following were
    observed:

    1. Aggregates delivered on site for subbase does not conformed on Item 200
    Material Requirements.
    2. There were no visible Vibratory Roller and Motorized Road Grader on site.

    This is contrary to the contract plans and specifications specifically on ITEM 200(1)
    Aggregate Subbase Course, and the pledge equipment as per POW wherein, on ITEM
    200(1) the Aggregate for subbase shall consist of hard, durable particles of fragments of
    crushed stone, crushed slag, or crushed or natural gravel and filler of natural or crushed
    sand or other finely divided mineral matter and the subbase material shall conform to Table
    200.1, Grading Requirements Refer to DPWH Blue Book

    In addition, to fast track the project implementation specially the on-going subgrade
    preparation, all pledge equipments shall be present on site.

    In this regard, you are hereby instructed to make necessary corrections and action
    regarding to above stated deficiencies.

    Your usual cooperation and compliance to this finding is earnestly desired.
