Philippine Passport Requirements

1. Juan Dela Cruz, single, would like to apply for a passport for the first time. He is a
government employee – connected with the Department of Finance, Manila Head Office.
For the application of passports, Government employees can submit the following documents:

• Copy of the Prior Intimation to the controlling authority or administrative authority

• No objection Certificate from the controlling authority or administrative authority
• Identity Certificate from the controlling authority or administrative authority
• Confirmed Online Appointment
• Personal Appearance
• Accomplished Application Form
• Original and photocopy of Philippine Statistics Authority (PSA) Authenticated Birth
Certificate on Security Paper
• Any of the following acceptable IDs with one (1) photocopy

2. Lisa Mondragon is a 15-year-old junior high student at the University of Baguio. She plans
to apply for a passport for the first time so she can visit her parents who are currently in
Venice, Italy in December.
In case applicant is not accompanied by the parent/s during the application process:

• Applicant must submit a Special Power of Attorney executed by the parent/s

designating the minor’s companion to assist in the application process. The Special
Power of Attorney must be authenticated by the Philippine Embassy/Consulate if
executed abroad
• Passport or Valid Government issued ID of authorized adult companion
• Special Power of Attorney or Affidavit of Support and Consent must have a copy of
parent/s valid ID and/or passport attached
In case applicant is not traveling with parent/s:

• Applicant must submit DSWD clearance and Affidavit of Support and Consent
executed by the parent/s. The Affidavit of Support and Consent must be Consularized
by the Philippine Embassy/Consulate if executed abroad

3. Ms. Chelsea Barretto, married, an employee of the Gret’s Events Company is a first-time
passport applicant.
• Married Females (who are using their spouse's last name) must also present Original and
submit photocopy of PSA Authenticated Marriage Contract on Security Paper or Report
of Marriage

4. Rojan Mark Tan is a 12-year-old student from Baguio City National High School. He is an
illegitimate child but his father acknowledged him and supports him financially. He plans
to go abroad to visit his mother.
In case applicant is an illegitimate child in custody of the Mother:

• Personal Appearance of minor applicant and mother or authorized adult companion

• PSA Birth Certificate
• Passport or Valid Government issued ID of mother or authorized Adult Companion (click

• If applicant is not accompanied by mother during the application process, applicant must
submit a Special Power of Attorney executed by the mother designating the minor’s
companion to assist in the application process. The Special Power of Attorney must be
authenticated by the Philippine Embassy/Consulate if executed abroad
• If applicant is not traveling with Mother, applicant must submit DSWD clearance and
Affidavit of Support and Consent executed by the mother. The Affidavit of Support and
Consent must be authenticated by the Philippine Embassy/Consulate if executed abroad.

5. Jacob Noah Rivera is a 2nd-year BSTM student from the University of Baguio. He is a first-
time passport applicant.

6. Miley Hemsworth is divorced from her husband. Her previous passport bears the family
name of her former husband. She plans to renew her passport and change the name with
her maiden name.
If an Applicant's marriage has been dissolved and wants to revert to her maiden surname

• Applicant must submit original and photocopy of PSA Birth Certificate and acceptable IDs.
• Applicant must submit original and photocopy of the annotated PSA Marriage Certificate
(MC) or Report of Marriage (ROM) stating that the marriage has been dissolved; OR
• Your previous passport
• Your birth certificate
• A judicial separation court order, separation agreement, or divorce decree
• Personal appearance of the applicant
• Accomplished application form for passport renewal
• Photocopy of the data page of the passport of husband and wife. Bring the original for
sighting.

7. Marikit Magtalas, single, is a 36-year-old employee of the University of Baguio. She plans
to apply for a passport but found out that her name is not registered with the Philippine
Statistics Authority (PSA).
If applicant has lacking data in Birth Certificate or Report of Birth

• Applicant must present original and submit photocopy of proof of filing of supplemental
report from the Local Civil Registrar (LCR)
If applicant has discrepancy in data in Birth Certificate / Report of Birth and other documents

• If the discrepant data is in the BC, the applicant must submit annotated birth certificate
authenticated by PSA reflecting the corrected entry. If the discrepant data is in the other
documents, the BC will be followed.
• Discrepancy in first name - Applicant must submit the original and photocopy of the
petition and supporting documents for correction of first name filed with Local Civil
Registrar (LCR) or consulate.
Visa Assistance
1. Lou Binay is planning to go to Japan alone for a vacation. However, she just realized that a
visa is required for Filipinos going there. Ms. Binay approached your company to seek visa
assistance and for the requirements, she needs to submit.

2. Erika Ancheta and Mr. Ken Ancheta plans to travel to Canada to visit their only daughter
who is working as a medical doctor in one of the government-owned hospitals in
Vancouver. They are senior citizens and are retired government employees. Mr. Ancheta is
an ex-Congressman and Ms. Ancheta is a retired Regional Director of the Department of
Tourism. Discuss to them in detail the needed requirements for them to apply for a
Canadian tourist visa.
• Valid Passport. Must have at least one-year remaining validity. Scan the bio-page;
• Completed Form IMM5257. This is the Application for Visitor Visa Form. Fill out the items
digitally and truthfully. (This only works on Internet Explorer and the latest Adobe Reader
version);
• Completed Form 5645. This requires your Family Information. It must be filled out digitally as
well;
• 2 recent passport size photos (35mm x 45mm). It must be taken within the last 6 months.
• Proof of income/financial capacity. (Bank Certificate/ Bank Statement for the last 4 months).
• Affidavit of Support. Only applicable if somebody else is sponsoring your trip. Also, attach
his/her proof of financial capacity;
• Travel history. A scanned copy of your visas (valid or expired) and your entry/exit stamps.
Make sure to just put everything in 1 file;
• Purpose of Travel. Your flight reservation, hotel reservations, and daily itinerary.

 3. Luis Fernandez is a father to Lorraine. Mr. Fernandez is not legally married to the mother
of Lorraine. He would like to process his daughter’s passport and visa because he wants
to bring her with him to Seoul, South Korea to watch the concert of Black Pink. Explain
