Dear LYRIA FREEMAN

FAMMILY OUTREACH Health System is pleased to offer you the position of Case Manager. We are very
excited about the potential you and your experience bring to the table. We anticipate your start date to
be 11/15/21

As discussed during your interview, your main office will be based out of perry where our Adolescent
MHP department is located. You will report directly to Mrs. Deborah Cawley as your immediate
supervisor. Once your complete orientation you will begin training and working in your position as Case
Manager.

If you decide to accept this offer of employment, your employment job responsibilities shall be as
follows:

Evaluate individual and overall family performance on a regular and ongoing basis; use collected data
and information to make needed improvements including adjustments as needed to caseloads and work
assignments to maximize quality of care, and staff productivity.
Ensures that each family and program goals (program, financial, personnel, professional development,
and trainings) are met in accordance with established contract program policies and procedures.
Actively participates in the development of program budgets and monitors program performance
according to set budget guidelines. Actively monitor assigned supervisees' performance goals and
professional development in accordance with agency and contract expectations.
Conduct semi-annual performance evaluations for the counseling team; proactively identify and resolve
performance issues. Recommend families as appropriate for recognition or professional development.
Assures that families and program goals (clinical, financial, governmental, professional development.
Start: You will be scheduled to begin work on11/15/2021 at 8:00Am.

You will have a beginning annual salary compensation of $17.50Hr

As an employee, you will also be reimbursed for the following out-of-pocket expenses, or as current
policy may dictate at the time.

- Mileage on personal vehicle at $0.85 per mile


- Meals, not to exceed $20.00 per meal, per person
- Meal reimbursement may include alcohol beverages, which shall not exceed a maximum of 2 per
meal
- Lodging shall be reimbursed at a maximum of $95.00 per night

After meeting the minimum 10days waiting period, you will be eligible to enroll and participate in the
following Company benefits in accordance with our policy and procedures, which may change from time
to time.

In accordance with Company policy, you will also be entitled to receive 30 days of paid vacation time per
year.

You or the Company may terminate the employment relationship at any time. Your employment shall be
deemed as voluntary in nature and shall be considered employment at will and both parties
acknowledge and agree that employment is not set for a specific duration of time.

If you decide to accept our offer of employment, and if you agree with the above terms and conditions,
we will formalize our discussion with an employment agreement that shall be presented to you on your
first day of employment. This letter itself does not constitute an employment agreement, only an offer
of employment.

Please confirm your acceptance of this offer of employment by 11/04/2021. I will be contacting you on
11/05/2021 to confirm your start date and to answer any questions you may have at that time.
