1.0 Man Overboard
Response of Deck Officer on Watch
□ Steer the vessel hard to the side where a person fell overboard
□ Start the second steering motor
□ Start Williamson Turn
□ S/B eng. And S/B full speed
□ Throw a lifebuoy, with self-igniting light or self-activated smoke signal over the side where a
man fell overboard
□ Sound the emergency alarm
□ Report to the Master
□ Inform the Engineer on watch
□ Continuously measure the vessel’s position
□ Plot the position of the lifebuoy thrown

Command
□ Announce “Man Overboard, Mobile team 2, proceed to bridge”, then order rescue boat station
□ Maintain lookout using binoculars
□ Maintain lookout via radar
□ Instruct the On Scene Leader to prepare the lowering of rescue boat
□ Carry out rescue preparation
□ Continue Williamson turn towards where the person fell overboard
□ Call the attention of nearby vessel through VHF

□ Sound the emergency alarm signal

□ Place staging area
□ Navigate the vessel in the way that the flame and smoke will flow downwind
□ Instruct wearing of fire protective gear and of preparing fire hose
□ Start up the fire pump

□ Prepare lifeboats and medical kits
□ Call the attention of nearby vessels via VHF

3.0 Oil Spill

□ Inform the barge to stop bunkering
□ Close all manifold valves
□ Stop ballasting work and close all valves concerned

□ Announce oil prevention station
□ Conduct a communication check/test

□ Instruct wearing of fire protective gear and of preparing fire hose

□ Open “SOPEP” Chapter III Control of Discharge
□ Pump remaining oil in the pipe line into another tank
□ Immediately transfer overflowed oil from the tank into another tank that has extra space
□ Request the company to conduct calculation of hull strength, stability and etc.
□ Lower liquid level of all tanks when oil leaking tank could not be determined

4.0 Piracy
Response of Deck Officer on Watch
□ Seal off means of possible access
□ Apply adequate lighting and patrols should be equipped with flashlights
□ Fire hoses should have constant supply of water to be rigged stern of the vessel
□ All crew members should proceed to secured areas

Command
□ Transmit Piracy Alert stating the vessel name, position and incident.
□ Avoid violence
□ All should muster at a secured area until the danger is over
□ Maintain continuous communication with E/R via portable walkie talkie
□ Check the vessel and crew for any loss or damage and inform the local port authority and the
company with the incident occurred.

5.0 Grounding
Response of Deck Officer on Watch
□ Halt the main engine
□ Sound the emergency alarm
□ Report to the master
□ Inform everyone on board of the grounding incident via public address
□ Switch to manual steering
□ Call the attention of nearby vessels
□ Notify the Engineer on watch
□ Transmit safety or urgency communication on channel 16 of the VHF
□ Stop the power and control units for the steering gear
□ Record the time of incident and ship position

Command
□ Make a roll call and check if there are any injured or missing personnel
□ Check the location and situation of the grounding incident
□ Check if there are any occurrence of fire
□ Check for occurrence of oil spill, if there are any, check its extent
□ Check for occurrence of flooding, if there are any, check its extent
□ Determine the pumping capacity
□ Check if there are any damages
□ Announce necessary stations
□ Place staging area
□ Check the sea bed and the status of grounding
□ Instruct the Back-up team to prepare lifeboats
□ Create a direct report to the Marine Administration Manager
□ Discuss with the company the possibility of refloating under own power
□ Make a request for salvage

LNG Team
□ Monitor the gas detector
□ Monitor Cargo Tank Pressure
□ Discharge Ballast Water

Engine Room Team


□ Check for any damage on the main engine, shaft system and other components related to the
engine
□ Pump the out-flooding water

Mobile Team 1
□ Check for occurrence of flooding, if there is, check the location
□ Check for any damage and its extent
□ Conduct sounding on all tanks and bilges
□ Read all record of the vessels draft from stern to bow and midship
