    AFFIDAVIT

    I, ALEX B. BALINGASA, of legal age, Filipino, single, and with


    residential address at 222 Avenida Veteranos St., Tacloban City, after
    having been duly sworn to in accordance with law, do hereby depose and
    state that:

    1. That I am the next of kin of Eduardo Balingasa, who died


    sometime in October, 2018;

    2. That the decedent did not leave a surviving spouse;

    3. That no personal representative has been appointed for the


    decedent’s estate in this state or elsewhere and no application for
    such an appointment is pending in this state or elsewhere.

    4. That this affidavit is made in support of the undersigned’s request


    for the release of birth and death certificate of Eduardo Balingasa
    at the City Civil Registrar of Tacloban City.

    Further, your affiant sayeth naught.

    IN WITNESS WHEREOF, I have hereunto affixed my signature this
