INFORMED CONSENT FORM

Information and Purpose: The interview for which you are being asked to
participate in is about “EXPERIENCES OF ADVENTIST ELEMENTARY
TEACHERS IN TEACHING ARALING PANLIPUNAN”. The main purpose of this
qualitative study is to explore the experiences of teachers teaching Araling Panlipunan
in Elementary grades.
Your Participation: Your participation in this study will consist of an interview
lasting approximately 30 minutes. You will be asked with a few questions about your
actual experience as an Araling Panlipunan teacher. You are not required to answer
all the questions. You may pass on any question that makes you feel uncomfortable.
At any time you may notify the researcher that you would like to stop the interview
and your participation in the study. There is no penalty for discontinuing participation.

Benefits and Risks: The benefit of your participation is your help to other Adventist
teachers by being aware of the struggles. The result will be used by the researchers to
as data for the research study. There are no risks associated with participating in the
study.
Confidentiality: The process will be through face to face. Your name will not be
divulged in the presentation of the result. However, your name will only be reflected
in this consent form. The researcher will not share your individual responses with
anyone except in the report in which your name will not be mentioned.
