Cardiac Auscultation
In our RLE today, our professor taught us the cardiac auscultation. Our clinical instructor
also demonstrated how to find the intercostal spaces, where to put the bell of the
stethoscope, and how to properly conduct a cardiac auscultation. After the discussion we
are tasked to listen to the heart sounds of our partner using the bell of the stethoscope and
get their radial pulse. The noises made by the beating heart and the blood flowing through
it are referred to as heart sounds. I have learned that there are four heart sounds that
occur in sequence with each heartbeat in healthy individuals. These are the first (S1),
second (S2) heart sound, third heart sound (S3) and the fourth heart sound, or S4.

First & Second Meeting

On our first and second meeting in NCM 103: Fundamentals of nursing skills
laboratory (RLE), Sir Modesto Fucio oriented us about the subject and he also
discussed different medical terminologies that would be very useful for us. It is
important to know those medical terminologies because we will be able to have a
clear understanding of information during diagnostic and treatment process of
patients. We had a lot of fun during the discussion. There are no dull moments
because the topic or lesson was very entertaining, and we are able to learn many
things while enjoying the discussion.

Body Mechanics

On the pictures below, I am demonstrating the different body mechanics. Body


mechanics is the safe use of the body to bend, carry, lift, and move things and
people utilizing proper posture, alignment, balance, and movements. Safe patient
handling and the application of body mechanics concepts safeguard both the
patient and the nurse. I performed the proper way of standing, sitting, walking, and
lifting or picking up of objects. Proper body mechanics is important because it
maintains good body posture, and it prevents muscle strain, injury, and fatigue.

Positioning and Draping

On the pictures below, I am demonstrating the proper positioning and draping to a


patient. It is important to know how to properly position the patient on the bed
because you will be able to assist the patient to assume a position that would best
afford adequate examination of a certain part of the body. While, the draping
provides comfort and privacy for the patient during the procedure.

Ear Assessment

In the assessment of the ears, I inspected the external and internal ear
structures first before performing the general hearing observation and
equilibrium tests. The different tests include the Whisper, Weber, Rinne, and
Romberg test. After the whole procedure, I recorded the findings and the
abnormalities that were observed.

Eye Assessment

In the eye assessment, vision test, extraocular muscle function test, and inspection
of external and internal eye structure are included in the procedure. On the
pictures below, I am conducting a vision test using a Snellen chart for distant visual
acuity. I checked the patient’s eyes for corneal light reflex or the parallel alignment
of light reflection on corneas using a penlight. I also performed the accommodation
test to check whether the pupils of the patient constrict. After the whole procedure I
documented the findings and abnormalities that were observed.

First week

On our first meeting in NCM 101: Health Assessment skills laboratory (RLE), Ma’am
Yolanda Rustia discussed the assessment of the ears and eyes. The different
external and internal structures of the ears were discussed. The equipment used
during the assessment were also mentioned. The different tests were also tackled
during the discussion.
