PROCEDURE FOR GLASS BREAKAGE/ GLASS POLICY

Purpose:
To lay down a procedure to remove the hazard of the broken glass on site, including lights &
windows. Also to establish a ‘Glass Policy’.

Scope:
The procedure is applicable to all the glass windows, lights and glass used in the warehouse,
production block A and clean room-1 (where the raw material or the product is exposed) at
Sri Vyjayanthi Labs Pvt Ltd.

Definitions:
NIL

Responsibilities:
Safety Executive shall be responsible to perform the monitoring activities as per the
procedure.
Safety In-charge shall be responsible for ensuring that all the records are complete.
QA In-Charge shall be responsible for compliance of the procedure and the Glass Policy.
All the employees must be responsible in notifying the glass breakage immediately by
understanding the potential risk associated with the glass breakage hazard.
Management shall be responsible to ensure that Glass Policy is adhered and Practiced.

Procedure:
Glass Policy Statement:
 Glass shall be eliminated in the process area, wherever possible.
 New Equipment’s / instruments shall be specified without the use of glass
components wherever possible.
 For existing equipment’s, alternative suitable components shall replace the glass
components if possible.
 Where Glass does exist, consideration shall be given to protect the glass from
breakage or spillage to prevent the product contamination.
 Unless for maintenance or replacement, the transfer of glass in to the process area is
prohibited.
 A risk assessment will take place to determine the potential product safety risks and
determine an appropriate inspection frequency.
 Glass Inspection will take place at regular intervals, as determined, in accordance with
risk assessment.
 Glass breakage incident shall be considered as potential risk and proper investigation
and immediate actions shall be taken to eliminate the potential risk and actions will be
taken to eliminate the possibilities of recurrence.
 All the employees must be aware of the GLASS POLICY and Procedure for Glass
Breakage. Both must be readily available.

Areas to be considered for implementation of Glass Policy:


All the areas where the raw materials/ finished product is exposed, be it during dispensing/
in-process/ sampling/ packing shall be considered in the scope of the Glass Policy.
The areas are:
Warehouse (WH)
Production Block (PB)
Clean Room (CR)

Finished Good Warehouse (FG)

Numbering of Glass/ Light:


The numbering shall be provided to all the windows/ lights where the potential risk is
observed.
The numbering shall be as follows:
For Windows it shall be WD/XX/YY
Where WD is Window
XX is Area (WH, PB,CR,FG)
YY is the sequence number (01,02,03,….)
For Lights it shall be LG/XX/YY
Where LG is Light
XX is Area (WH, PB,CR,FG)
YY is the sequence number (01,02,03,….)
The numbers shall be mentioned on the windows and next to the lights for monitoring.

Controls:
No glass items shall be used in the processing areas where potential risk is observed.
Shatter proof bulbs or bulbs with shields are shall be used.

Lights/ windows shall only be replaced during non-production hours when there is no
exposed product.
If a replacement is needed during production hours, all product/ packing materials must be
covered and moved away from the fixture area to prevent accidental contamination.
The PH shall be checked by using PH Meter away from the reactor by collecting the sample.

Glass Breakage:
All the glass breakage shall be immediately reported to the in-charges.
The breakage or loss of eye glasses and contact lens should also be reported to the shift in-
charges.
In the event of glass breakage, the following must be followed to ensure the product safety:

The production in the area must be stopped immediately.


All the products in the immediate vicinity must be quarantined.
The area at risk must be isolated by placing the safety cones.
The area in-charge and safety head should be immediately notified.
No items/ equipment’s/ glass should be removed until the site has been examined by the
safety head.
All the employees should leave the immediate vicinity by inspecting the shoes for the glass
pieces, to ensure no further contamination.
The affected staff PPE should be changed, if any. And the affected PPE should be discarded
to avoid further cross contamination.

Cleaning of the glass breakage should be done as follows:

A nominated employee on instruction shall pick/ sweep up all the glass pieces by wearing
appropriate protective clothing and gloves.
Broken glass will be collected in to the poly bag.
All the contact surfaces, machinery and the floor area should be wiped using a damp cloth.

Brushes, brooms and cloths used to collected the broken glass must be disposed along with
the poly bag and PPE used immediately.

Area must be checked by both area in-charge and the safety in-charge before resuming the
work.
The repair of the broken glass item should be done to ensure no further damage is caused.
Work order should be raised accordingly.
The safety in-charge should ensure that all the action is recorder in the Glass Breakage
Record. (Annexure-1)
In the Glass Breakage Record:
Date/ Time (Ex: 02/07/2022), Incident number (As per sequence, 001,002, 003….) and
Location/ Area (Production Block A/ Solid Raw Material dispensing room….) shall be
mentioned.
Glass Product Name/ Identification Number shall be recorded. (Example: Window Glass /
WD/WH/02 or Light Glass LG/PD/10)
Description of the Incident shall be mentioned.
During the glass incident if there is any injury happened, name of the person and details of
the injury on how the injury happened shall be recorded.
During the glass incident, Any product in the immediate vicinity shall be verified and if
any cross contamination should be examined and recorded.
Actions on the Incident- Root Cause Analysis and Corrective/ Preventive actions shall be
performed and recorded.
Both the Area In-charge and the Safety In-charge should sign and date the Record to
resume the further operation in the incident area.

All the Glass breakage Records are need to be maintained accurately and investigation of the
each incident should be performed properly to eliminate possibilities of glass incidents in the
process areas.

Monitoring and Self-Inspections:

A list of all the glass items/ glass windows and lights, indicating the location in the each
process area shall be maintained by the safety department. ‘ Master list of Glass/ window/
light’.
Daily monitoring of the equipment glass shall be done by the production In-charge and the
shall be recorded in the Daily Equipment Glass Log.
Monthly monitoring of the glasses shall be done by the safety department and the same shall
be recorded in the Monthly GlassMonitoring Record.
During the monthly monitoring each glass/ glass window and light shall be checked as per
the Master list and shall confirm the intactness.
