RELEASE, WAIVER AND QUITCLAIM
I, ___________, of legal age, Filipino, married and a resident of

____________________________________________________, after having been duly
sworn to in accordance with law, do hereby depose and say:
1. That I am the same ___________ who has been an employee of

____________ from _______ to _____, and has retired therefrom upon reaching
within the age of __________; thus, became eligible for ________ retirement;
2. That I affirm that during the period of my employment with ________, I

have received from the Company all of my accrued salary/wages and other benefits,
including that of the Labor Code and of Company Policies, from the date of my
employment to the date of my retirement, as well as all amounts that are now or in the
future may be due me for the services which I have rendered during my employment
with the Company;
3. That I have received and been paid the amount of

_______________PESOS (P______________) by way of Cash (or Check No.
______________), representing full and complete settlement of my retirement pay
and other monetary claims from the Company.
4. That in consideration of all sums, moneys, benefits and advances I

received, I hereby now release, abandon, remise, forever discharge, and quitclaim any
or all claims which I, my heirs, successors, and assigns may in any event have against
the Company, its principals, assigns, subsidiaries and affiliates, and their respective
stockholders, directors, officers, employees, representatives, or agents, from any and
all claims, demands, cause of action, or case of whatever nature as may be filed by
me or any other person arising from or relating to all incidents of my previous
employment with the Company or the cessation thereof;
5. That I further acknowledge that should I be awarded any claim under any
law or any order of a competent authority, the above-mentioned considerations shall
be considered full and final settlement of any and all such claims;
6. That I shall not at any time or in any manner whatsoever, directly or

indirectly, engage in any activity nor divulge any information, data or documentation
(whether confidential or otherwise), which I may know or possess relating to the
business of the Company, including but not limited to, any of its clients or any other
confidential information concerning the business of the Company, its manner of
operation, its plans, processes or date of any kind, or that may in any way or manner,
directly or indirectly, prejudice, injure or adversely affect the business or reputation
of the Company which I acquired, learned or received in confidence during my
employment;
7. That should I violate this undertaking, I shall pay to the Company

liquidated damages equivalent to double the amount I received hereunder, without
prejudice to the Company’s right to file the appropriate cases or charges and seek
such damages and losses that it may have suffered or incurred as a result of my
breach of this undertaking;

8. That I shall render every cooperation necessary and obligate myself to turn
over and account for all Company records, books, properties, and assets in my
possession.
9. That I have read this document and that I have understood its contents.
The entire release, waiver and quitclaim hereby given is made by me willingly,
irrevocably, unconditionally and voluntarily, and with full knowledge of my rights
under the law;
10. That I am executing this affidavit of waiver/quitclaim to attest to the truth

of all the foregoing facts and for whatever legal purposes this may serve.
