1. Endorsement from the Schools Division Superintendent as proof
that the documents have passed the Schools Division Office
2. Inspection report and recommendation by the Division Private
Schools Supervisor/ Team
3. Application Letter of Recognition
4. School Bond/Certificate of Warranty Bond
5. Letter of Request/ Board Resolution stating the purpose of the
application, the school year and the course offered
6. Articles of Incorporation and By-laws duly registered with the
Securities and Exchange Commission
7. Copy of the Certificate of Title of the School Site or Deed of
Donation (at least 1 hectare for 50-100 pupils)
8. Documents of ownership of the school building
9. Certification from the Barangay that NO Private school is located
within the 2km radius from the school to be established
10. Proposed School Curriculum Standards and requirements
11. Proposed Tuition and other School Fees
12. Proposed Annual Budget and Annual Expenditures for the school
year
13. Enrolment report
14. School Calendar
15. Classroom Program prepared and certified by the School
Head/Administrator;
16. Teacher Program individually prepared and certified by every
teacher of the school (to be shown to, and collected by the MBHTE-
Regional Office TWG during their ocular inspection)
17. Members of the Board of Trustees and Lists of School
Administrator/s
18. List of Academic and Non-Academic personnel
19. List of laboratory facilities, equipment and furniture
20. List of Library Holdings
21. Original pictures (classrooms, school buildings, restrooms,
laboratories, library, medical and dental clinic, school canteen,
playground, school stage, flagpole, principal’s office, faculty room,
computer/ multimedia room;
22. Inspection report of the team.
23. Recognition Fee of 2,000 paid before the release of the permit
