Project Monitoring Report Form 2022
The table above is the Project Monitoring Report which indicates the intervention projects,
objectives, date of monitoring, accomplishment/status as well as the issues and challenges
encountered. The enumerated projects are all in the on-going status because they have
been implemented and some are presently on testing period so that issues/concerns can be
noted and addressed immediately for recommendations and further improvement before it
will be implemented for the next school year.
4. 1 Monitoring and Evaluation Strategies
Sta. Cristina Elementary School has an institutionalized quarterly School Monitoring,
Evaluation and Adjustment (SMEA) conference. The school MEA is headed by the School
Head (SH) and vice chaired by the school M and E coordinator with the help and support of
the members including the GPTA and SCESTA President. The SMEA conference is an
outcome driven, evidence based and participatory in process activity. The main goal of this
conference is to help the school achieve its goal through presenting valid, timely, useful and
informative data about the ACCESS, QUALITY and GOVERNANCE.
4.2 Monitoring and Evaluation Activities
The School will be implementing the following activities
Quarterly SMEA Conferences- This event is scheduled every 1 st week of April, July,
October, and January. The salient points which will be discussed in the conference
are ACCESS such as enrollment, intake rate, learners who are not participating in
school activities or PARDOs, achievement rate, PPAs accomplishment and status,
MOOE utilization, issues, challenges, bottlenecks, opportunity for improvements,
and successes and etc.
Meetings and SLAC- These activities are intended for the provision of technical
assistance to the PPA proponents.
Monitoring and Evaluation of Learning and Development ( L and D) activities. ( All
year round)
