    1. I, the undersigned, have fully understood that Pfizer Limited ("Pfizer") is authorized by me to
    collect and process the following classes and categories of my personal information:

    2. I am fully aware that the aforesaid information may include sensitive personal information
    about me, including my financial details and gender. The categories of information set out above
    shall hereinafter be referred to as my “Personal Information”.

    3. I consent to my Personal Information being used by Pfizer for the following purposes (each
    hereinafter referred to as a “Purpose” and consent to Pfizer retaining my Personal Information
    for the corresponding retention periods set our herein:

    herein and be bound by the terms of this consent form.

    9. I agree that Pfizer may notify me of any updates to the updated purposes, retention periods,
    transferees specified herein (so long as such updates do not have an adverse impact on my rights
