    I, Smt Anindita Chakraborty,W/O- Sri Abhisek Chakrabarty,


    residing at3/E –Sandhya Arati Apartment, Sekhpura,Church
    Road,P.O- Medinipur, P.S- Kotwali, Dist- Paschim Medinipur, aged
    about 41 years, by caste-Hindu, by profession – Teacher, do hereby
    solemnly affirm & declare as follows,

    1. That before marriage my surname name was “Ghosh” which


    has been recorded in all my school documents including in
    Admission card of council for the Indian School Certification
    Examination, New Delhi and also in my Pan Card vide no-
    AKLPG8656M and also in the appointment letter issued by Additional
    Dristrict Inspector of School(SE),Kharagpur Sub –Divisions, Paschim
    Medinipur.

    2. That after marriage with Abhisek Chakrabarty on 01/05/2006,I am
    writing my surname “Chakraborty” which has been recorded in my

    3. That my name has been also recorded as Anindita Chakraborty


    (Ghosh) in my Marriage Certificate issued on 14/08/2006.

    4. That I am swearing this affidavit just to declare that Smt Anindita


    Chakraborty, W/O-Sri Abhisek Chakrabarty, and Anindita Ghosh
    D/O-Lt. Tapan Ghosh and Anindita Chakraborty (Ghosh),W/O-
    Abhisek Chakrabarty, all of residing at 3/E Sandhya Arati
    Apatment,Sekhpura,ChurchRoad,P.O.-Medinipur,P.S.-Kotwali,Dist-
    Paschim Medinipur, are the same and identical person.
