Chief Complaint: Pain on the left knee, swelling on left thigh, fever 5 days after fall
Medical Impression: Fracture Closed Distal Femur Left

B. Nursing History (Gordon’s 11 Functional Health Pattern)

I. Health Perception – Health Manage Pattern


According to the client, before her injury, she has been feeling good, can perform her work daily, and
has absence of any illness or diseases. She has never encountered any health problems in her entire
life and viewed herself as a healthy individual. The client’s only cause of hospitalization is due to the
injury she sustained after falling in their comfort room.

II. Nutritional – Metabolic


The client said that their family eats three times a day, with snacks in between. Their diet consists of
fish and vegetables. They rarely eat beef since their family’s favorite meat is fish. According to the
client, she does not take any vitamin or food supplement to boost her health. The client loves to drink
water and she drinks water whenever she gets the chance to drink. She said that it is impossible for an
hour to pass by without her drinking water. However, the client loves to drink Royal, a soft drink, and
Tang, a powdered juice. The client said that she can consume one liter of Tang in one sitting.
According to the client’s daughter in law, the client gained a significant amount of weight recently
due to her diet, and lifestyle.

III. Elimination Pattern


The client’s normal frequency of defecation is at least once a day. During assessment, the client had
already defecated on her diaper prior to my arrival. She usually voids approximately four times a day.
Accordingly, she does not have any problem that involves elimination, she have never used any
laxative nor stool softeners.

IV. Activity-Exercise Pattern


Before the client’s injury, the client said that she is very energetic and loves to move around. She
loves to clean their house and move their furniture often. She considers these activities as a form of
exercise, To keep her entertained if she is doing nothing, she loves to watch videos on YouTube while
munching on snacks. However, when the accident occurred, she was on bed rest the whole time. She
can only perform limited activities due to her condition. The following are her perceived ability for:

Sleep-Rest Pattern
The patient usually has a complete hours of sleep. Accordingly, the client sleeps for 7 to 8 hours
nightly. The night prior to my assessment, the client slept from 11:00 P.M. to 6:00 A.M., she was able
to sleep for 7 hours. She does not have any difficulties going to sleep and has never used any
medication to promote sleep.

Cognitive-Perceptual Pattern
The client is oriented to people, place, and time, and responses to stimuli verbally and physically. She
speaks Tausug but can also communicate using Tagalog. Her educational attainment is high school
undergraduate, and she is able to read and write without any problem. The client does not have any
hearing difficulties. However, she has a problem with her vision. She is wearing a prescription
glasses, and has a grade of 250 on both her left and right eye. She described her memory as sharp and
does not have any problem with it.

Self-Perception Self-Concept Pattern


The client described herself as mabait and matulungin. She wakes up every day and decides to be
kind to all the people around her, and help the people in need as much as possible. However, she said
that whenever she gets angry, which rarely happens, she tends to blurt out mean words that she does
not mean to say, and regrets saying them when her anger subsides. She said she has been gaining
weight recently due to her diet. She said that she consumes a lot of junk food every day, she loves to
eat Clover and drink Royal or Tang. The client is planning to reduce her weight after she recovers.
She said that she plans to exercise regularly and get more involved in activities that requires moving
her body as much as possible.

The client was able to express her feelings about her condition. She said that her daughter considers
this incident as a “blessing in disguise” since she does not rest when she is well, she loves to do
household chores and move heavy furniture to redecorate their home.

Role-Relationship Pattern
The client’s role is to be a mother to her biological children and in-laws, a sister to her brother, and a
wife to her husband. She takes good care of her family and has a close bond with them. It can be seen
during the assessment that the client and her daughter-in-law have a close connection with each other
since they love laughing around and they know each other very well, including the things that they do
during their free time. There are no conflicts among them, and everyone shares their ideas when it
comes to decision making.

Sexuality-Reproductive Pattern
The client claimed that she and her husband have been sexually inactive for a long time already due
to their age.

Coping-Stress Tolerance Pattern


As mentioned, the client and her family have a close relationship with each other. In times of
difficulties, they gather together to talk about the problem and discuss the different solutions they can
do to solve the problem. They make sure to be with each other in times of difficulties and make the
person with the problem less burdened. They do not take any medication to aid with their handling of
stress, they just make sure to be present for each other. Despite going through a lot of hardships in
life, they still get through it and handle it successfully.

Value-Belief Pattern
The client stated that her religion is Islam and has a very close connection with Allah. She prays
regularly and sacrifices her sleep sometimes to pray. She believes that Allah is the provider of help
and comfort in times of need, and showers her family blessings.
