    AFFIDAVIT OF CONSENT TO ADOPTION

    I, SHARON ECHIPARE MOSLEY, Filipino, of legal age, married and resident of 4828
    Post Oak Road, Anniston, Alabama, USA 36206, having been duly sworn in accordance with law,
    hereby depose and state:

    1. That I am the legitimate daughter of petitioner JOSEFINA CUYUGAN ECHIPARE and her
    deceased husband AQUILINO EDRA ECHIPARE and was born on November 3, 1978 in
    Caloocan City, Metro Manila, Philippines. Attached hereof as Annex “A” is a copy of my
    Birth Certificate as proof that I am petitioner’s legitimate daughter;

    2. That I am aware that my mother JOSEFINA CUYUGAN ECHIPARE is in the process of


    adopting SHABELLA JOSEPHINE PALAYPAY, illegitimate child of my cousin DAYANARA
    CUYUGAN PALAYPAY who was given sole care and custody of the said
    unwanted/illegitimate child;

    3. I hereby signify my written consent, freely, willingly and voluntarily to the adoption by


    my mother of SHABELLA JOSEPHINE PALAYPAY in accordance with law;

    4. That I am ready and willing, freely and voluntarily, to  acknowledge and be bound
    legally by any rights the adoptee may be entitled to under the Family Code of the
    Philippines and the Civil Code of the Philippines including provisions on succession and
    inheritance; and such other rights and privileges that may operate in adoptee’s favor
    under the law after the adoption proceeding had been instituted and an order of
    adoption be issued;

    5. I shall be supporting my mother JOSEFINA CUYUGAN ECHIPARE in her adoption process


    and would assist in ensuring that the baby’s own interest and benefit shall be
    paramount and knowing full well that that the adoptee shall be taken care of and
    provided with strong emotional support and guidance while growing up and such other
    needs such as tender love, care, affection and education just as my mother had done for
    me, my deceased sibling and my son SEAN YVOR GABRIELLE ECHIPARE;

    6. That I execute this affidavit to declare my consent to the adoption of baby SHABELLA


    JOSEPHINE PALAYPAY as well as to the truth of the foregoing facts.

    IN WITNESS WHEREOF, I have hereunto set my hand this 24 th day of February, 2020 at


    Manila, Philippines.
