ABG Interpretation Made Easy

Remember this chart above and the

Ph comes first Normal Value is 7.35 – 7.45


Less than < 7.35 is Acid,
Greater than > 7.45 is Base

PaCO2 comes Second hence he 2 in PaCO2. PaCO2 Represents the Lungs


Normal Value is 35 – 45 (7 goes to heaven using Ph as
reference).
Less than < 35 is Base
Greater than > 45 is Acid,

(Bi Carb) HCO3 come Thirds hence the 3 in HCO3.


Normal Value is 22 – 26
Less than < 22 is Acid,
Greater than > 26 is Base
Step 1
pH comes First

Step 2 Match pH with a partner


Example: pH is 7.2 and PCO2 50 HCO3 23

Ph Match with PaCO2 making it Respiratory Acidosis


Step 3-A Compensated or Uncompensated

Ph Match PaCO2 making it Respiratory Acidosis no movement with HCO3 which makes
this Uncompensated Respiratory Acidosis

Step 3-B Fully Compensated or Partially Compensated

The body will try to balance Acid and Base to return to a Normal State like a Tug of war.
If pH is Normal it is a Sign of Full Compensation If the pH is not Normal it can be
interpreted as Partially Compensated. Let’s look at some examples.

Example 1: pH 7.30, PaCO2 52 , HCO3 40

pH is Low 7.30 making it Acidic, PaCO2 52 making High (Acid Match with pH), HCO3 40 is
making it High and on the Base State.
Interpretation: Respiratory Acidosis Partially Compensated as the Kidney is trying to
Compensate (partially as the pH is still not within normal Range).
Example 2: pH 7.55, PaCO2 47, HCO3 is 35

pH is High 7.30 making it Base, PaCO2 47 making High, HCO3 40 is making it High (Base
match with pH) and on the Base State.
Interpretation: Metabolic Alkalosis Partially Compensated
Note: the lungs is trying to compensate with the base state but since the pH is still not
normal it is only partially compensated.

Example 3: pH 7.36 , PaCO2 50, HCO2 29

pH is 7.36 Normal , PaCO2 is Acidic 50, HCO3 29 is Base (No Ph Match?)


pH is Close to Acid meaning it matches with PaCO2 making this Respiratory Acidosis
Compensation: Ph is Normal , PaCO2 is Acidic 50, HCO3 is High to Compensate
Interpretation: Respiratory Acidosis Fully Compensated
