Discharge Plan (METHODSS)

Name of Patient: G.A.

M: Medications: Educated and advised the patient to adhere to the prescribed


medication of his physician which are the Olanzapine( tx of treat mania or mixed
episodes that is part of bipolar disorder) Biperidine (drug-induced movement disorders,
improving muscle control and reducing stiffness, this medicine allows more normal
movements of the body as the disease symptoms are reduced)

and Carbamazepine (prevent and control seizure, anticonvulsant or antiepileptic


drug) following its ordered time of intake, frequency and dosage. Asked family
member to monitor patient and report presence of adverse effects. Giving
emphasis in avoiding self-medication and abrupt medication withdrawal.

E: Exercise/ Environment: Patient was advised to have a regular exercise


and perform stress reducing activities as tolerated. Avoid activities that are
potentially dangerous thus removing the objects that might inflict harm to
patient and significant others. Families are encouraged to provide a soothing
environment conducive for the recovery of the  patient in promotion of the
overall health status.

T: Treatment: Emphasized and ensured proper adherence with follow-up


check up as instructed by physician, aside from the oral supplements some of
the treatment and test that can be done to monitor the patient’s health and
mental status is the the regular laboratory to ensure that the medication where
effective and is absorb by the patients body system as well as the regular
psychiatric test. Promote positive self-talk and increased socialization by
encouraging family members and significant others to listen and address the
patient’s concerns/ mood/ feelings.

H: Hygiene: Advised patient and significant other to maintain a clean and safe
environment at all times.Encourage patient in performing self-care activities
such as bathing, brushing teeth, and combing hair, ask for family/ relative
assistance if needed, give emphasis in maintaining a good and proper hygiene.

O: OPD: Follow up check two weeks upon the discharge and on monthly basis
is advised to the patient. Refer patient to stay socially active and advise to join
some support group that could assist in his emotional state.Call healthcare
provider if signs and symptoms occurs again: inflicting self/ other directed

harm, unstable intrapersonal and interpersonal relationship, impulsivity,


recklessness.

D: Diet: Patient was advised to take diet as tolerated, noting the importance of
eating a well-balance and nutritious foods, with at least 8 to 10 glasses of
water a day.

S:Spiritual: In this time of pandemic, with the client being in the adolescence
stage, having very limited social interaction, provision of spiritual care is seen
as an essential tool in order to achieve a better coping mechanism and positive
outlook in life. Therefore it is vital to encourage patient to strengthen his
spiritual outlook and continue to seek God’s guidance and enlightenment by
asking for divine guidance and assistance in everything thus emphasizing the
importance of prayers in healing, going to church and attending Holy Mass.

S:Sexuality: Maintain good open communication and respect to the same and
opposite gender.

Conclusion:

In able to achieve the optimal health of the patient and appropriately deal with the
needs, management and treatment of a service user a knowledgeable, skilled
and competent health care team is a must. Promoting a therapeutic environment
conducive to patient's current health status is important to establish trust and
rapport, through the proper utilization of therapeutic communication techniques
and maintenance of non-judgemental attitude by allowing the patient to have
ample time to process and express his thoughts you will be able to help him
identify his strengths and weaknesses that will be vital upon the course of
treatment. The patient’s thought process and actions must always be monitored
in order to provide a tailor-fitted therapy and management of the mental health
disorder.Triggering factors should be identified and appropriate management
should be presented. Furthermore, there is a need to monitor the psychological
state, as service users are at an increased risk for self-violence toward self or to
others, which may lead to suicidal attempts if not attended immediately. Assisting
the patient to be aware of or deal with the disorder diagnosed is highly essential
in order to ensure an improved judgement and insight regarding his status.
Health teaching is extremely important for the patient. The student nurse must
help educate and demonstrate to the patient the essence of compliance and
adherence to the management established to avoid relapse of the condition.

Most especially, involving the immediate family members with the plan of care is
very much vital to prevent recurrence of disruptive family relationships and
establish a safe and trusting environment for the client.

Recommendation:

For the nursing students:

Learning is a process,always allow yourselves to explore and actively


participate in nursing procedures done during the shift. Maintain your
eagerness to gain more knowledge through practice during clinical duties.
Dedication and commitment to nursing profession are highly
recommended to all nursing students.

For the clinical instructors:

We are thankful for having ever dynamic and considerate instructors


during our clinical duties. We appreciate the time that they are rendering to
us and the knowledge that they are catering to all nursing students. We
recommend having more days to spend and learn with for us to gain more
knowledge through practice that we can apply inside and outside the
hospital. We salute you for the hard work and dedication you have made in
the nursing profession.

For our adopted hospitals:

We are grateful for the good accommodation and for letting us apply the
knowledge that we have learned in school towards this four day clinical
rotation at Batangas Medical Center – Acute Psychiatric Unit.
