    AFFIDAVIT OF NO PENDING CASE

    I, LEXTER OWEN B. APELADO, Filipino, of legal age, single and with postal
    address at #38 Dansalan St., Mandaluyong City, Philippines, after having been
    sworn in accordance with law hereby depose and state that:

    1. I am an employee of Printwell, Inc., a corporation organized and


    existing under an in accordance with the law of the Republic of the
    Philippines, with principal office and place of business at 38 Dansalan
    St., Mandaluyong City, of which I hold the position of HRMA
    Manager;

    2. To my knowledge and belief, there is no pending case proffered


    against Printwell Inc. before the Department of Labor (DOLE) and
    Employment and National Labor Relations Commission (NLRC);

    3. If I should thereafter learn that a case has been filed or pending, or


    that a complaint was proffered against Printwell Inc., I shall report
    the fact within five (5) days from knowledge to the
    Department/Commission the complete statement of the present
    status of the case;

    4. I am executing this statement for the purpose of securing a


    Certificate of No Pending Case from the NLRC, which shall be utilized
    by the company for its business related ventures and for whatever
    legal purpose it may serve best.

    IN WITNESS WHEREOF, I have hereunto affixed my hand this 11 th day of


    May 2015 at Mandaluyong City, Philippines.
