ASSESSMENT

Verify doctor’s order

Good morning, I’m Nicole your student nurse for today. I’ll be doing nasogastric tube feeding on you.

Can you state your name? Can you state your date of birth?

Are you allergic to any food? None.

I’m going to examine your abdomen using stethoscope and palpation to check the functioning of your GI
tract.

PLANNING

The materials needed are now gathered in the bedside table.

The formula is not expired, and the container is in good condition.

I’ll be feeding the patient in a room temperature to prevent gastric cramps.

I’ll be feeding you formula using the nasogastric tube attached on you and asepto syringe.

Close curtains and perform hand hygiene. Apply clean gloves.

I’m going to elevate your head 45 degrees to prevent aspiration.

IMPLEMENTATION

I’ll be putting towel above your chest.

I’m going to attach the syringe, and the tube should be clamped. Connect it.

Aspirate 5ml of gastric content. Reclamp the tube and check the appearance and PH of gastric content.

The ph reading should be 1 to 4.

Check the gastric residual volume now.

Assuming that the gastric content withdrawn is below 50ml, I will now proceed with the tube feeding. If
the gastric content withdrawn is 100ml, I must refer to the physician first.

Proceed with intermittent feeding


AFTER CARE

I will keep you in this position for 30 minutes, okay?

I’ll be pinning the tube to your gown.

state “I have washed the equipments after using it”

EVALUATION

Do you feel full? I will be assessing your abdomen using stethoscope, okay?

DOCUMENTATION

I’m recording the amount of feeding and the patient’s response to tube feeding.
