    AFFIDAVIT OF LOSS OF CERTIFICATE OF REGISTRATION OF


    MOTOR VEHICLE
    I, PETER D. TURTEM, of legal age, married, Filipino citizen and a resident of
    MB-460 Chinese Village, Puguis, La Trinidad, Benguet, Philippines, after having duly
    sworn to in accordance with law, do hereby and depose and state THAT:

    1. I am the owner and possessor of a Certificate of Registration (CR) No.:


    ______________ with an Official Receipt No. 1802364126 issued by the
    Land Transportation Office (LTO). Said motor vehicle is particularly
    described as follows, to wit:

    File No: 138000000248371 Plate No.: XEB930


    Make: Isuzu Classification: Private
    Type: Elf Fuel: Diesel

    2. To my recollection, sometime on 2018, when I was in my residence at Puguis,


    La Trinidad, Benguet, I have kept my car documents together with some
    important documents inside my car, when I look my documents inside my
    car, purposely to renew my registration with the LTO. I noticed that my
    Certificate of Registration (CR) was no longer inside my car which I kept it in
    a folder for safe keeping. Then and there I tried to search immediately in the
    premises to locate for the same but it was nowhere to be found thus, all my
    efforts proved futile.

    3. I have exerted due diligence to recover said Certificate of Registration but


    such effort turned futile since the same was never found or cannot be located
    until now. I am now of the honest belief that said Certificate of Registration
    (CR) is already lost and beyond recovery;

    4. The said Certificate of Registration was not confiscated by any Government


    authority or law enforcement agency as a result of any violation of law nor
    delivered or mortgaged to any person or entity for the satisfaction of any civil
    obligation;

    5. I am executing this Affidavit to attest to the truth of the foregoing facts and for
    all legal intents and purposes this may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 21st day of April
    2022, at La Trinidad, Benguet, Philippines.
