    I, Jessie Sacramiento Gabor, legal age, Filipino, and a resident of 117 Santolan Road,
    Quezon City. After having been duly sworn to in accordance with law, hereby declare that:

    I am the petitioner seeking correction of the clerical error in:


    My Certificate of Live Birth
    I was born on September 17, 1970, at Clinica Arellano, 1430 Doroteo Jose, Sta. Cruz,
    Manila.
    The birth was recorded under the registry number 2645 (i70) ,
    The clerical error(s) to be corrected is/are:

    12 Mothers surname was wrong Sarmiento Sacramento

    The facts/reasons for filling this petition are the following:

    For error No. 1: My middle name should be spelled as Sacramento instead of initial of “S” as
    it appears on my live birth certificate.
    For error No. 2: My mother surname should be spelled as Sacramento instead of Sarmiento
    as it appears on my live birth certificate.

    i). Philippine Health Insurance Corporation Member Data Record

    j). Social Security System Contribution Record

    k). Photocopy of Driver License

    l). Photocopy of Unified Multi-Purpose ID

    m). Photocopy of Quezon City ID or QCitizen ID Card

    I have not filed any similar petition and that, to the best of my knowledge, no similar petitions
    pending with any LCRO, Court or Philippine Consulate.

    I am filling this petition at the LCRO of Metro Manila


    in accordance with R.A. 9048/ R.A. 10172 and its implementing rules and regulations.
