MICROBIOLOGY
Specimen received: Sputum (Suspect new : 0mths)
Tests requested: GeneXpert Ultra, TB mic @, TB cult @, GeneXpert XDR CS @
@ Test referred to another NHLS laboratory

Real time PCR for M. tuberculosis (Xpert MTB/Rif Ultra):

PCR result Mycobacterium tuberculosis complex detected
Rifampicin (molecular) Resistant

This patient has presumptive MDR-TB. Please refer URGENTLY to an appropriate

treatment facility and refer to the Drug Resistant TB management guidelines.

Auramine O Stain:
Result (concentrated) - IUALTD Negative (No AFB observed in one length)

Molecular resistance testing for MTB: TB PCR DR-TB (Clinical Sample):

PCR result Mycobacterium tuberculosis complex detected
Isoniazid, INH (molecular) Low resistance
Fluoroquinolones, FLQ (molecular) Sensitive
Amikacin, AMK (molecular) Resistant
Ethionamide, ETH (molecular) Resistant

Interpretative comments:
1. Mutations associated with low-level Isoniazid resistance were detected.

2. No mutations conferring resistance to the Fluoroquinolones (FLQ) were

detected. This assay is able to detect 94% of FLQ resistance.

3. Mutations associated with Amikacin resistance were detected.

4. Mutations associated with Ethionamide resistance were detected.

NB. Please correlate with current genotypic Rifampicin result, to determine

the drug resistance classification. Phenotypic drug-susceptibility results
of additional second-line drugs to follow.
