Product Disclosure Sheet

Note: Please read this Product Disclosure Sheet before you decide to take up ProtectActive. Be sure to also read
the general terms and conditions.

1. What is this product about?


ProtectActive is a yearly renewable group term takaful coverage that provides one year protection for covered
Critical Illness as well as Accidental Death or Accidental Total and Permanent Disability directly related to the
participating sports. It also reimburse the Medical Expenses incurred per accident, which are directly related to
the covered participating sports.

2. What are the Shariah concepts applicable?


 Tabarru’: The amount of donation that the Person Covered willingly relinquish to help each other in the
event of misfortunes.
 Wakalah: The contract of agency where:
(i) Wakalah bi al-ujrah: the charge imposed upfront as Participant/Person Covered appoints Us to
manage Tabarru’ Fund.
(ii) Wakalah bi al-Istithmar: the charge imposed upfront as Participant/Person Covered appoints Us to
invest the monies.
 Ju’alah: Literally, it means compensation for a given service. Legally, it is a contract for performing a given
task against a prescribed fee in a given period. Ju’alah concept is used in a situation where Underwriting
Surplus is shared among the Person Covered and Us. Entitlement to Underwriting Surplus depends on
completion of work and delivery of result.
However, the portion distributable to the Person Covered will be channelled to any charitable organisation(s)
approved by Our Shariah Committee due to the short-term nature of this product and the expected amount
may be too small to be distributed to the Person Covered.
 Mudharabah: Profit sharing between Person Covered and Us on the investment return of Tabarru’ Fund
based on pre-agreed ratio of 50:50 respectively which is distributed after each financial year. The portion
distributable to the Person Covered will be channelled to any charitable organisation(s) approved by Our
Shariah Committee due to the short-term nature of this product and the expected amount may be too small
to be distributed to the Person Covered.
 Qard (Loan): A loan which is returned at the end of the agreed period without any interest or share in the
profit or loss of the business. For the purpose of Takaful, in the event of deficit in the Tabarru’ Fund, We will
arrange for Qard. The Qard is repayable from the future Underwriting Surplus of the Tabarru’ Fund.

Note: Tabarru' Fund is a pool of funds established for the purpose of solidarity and cooperation among the Person
Covered that is used to help all Person Covered in the event of misfortunes.

3. What are the covers/benefits provided?


The benefits under this plan are as follows:

Accidental Total and Permanent Disability (TPD) Benefit
Upon TPD due to accidental causes directly related to the
participating sports that occurred during the coverage period, the
sum covered will be payable. TPD must be certified by a Medical 5,000 10,000 15,000 20,000
Practitioner appointed by Us to have continued for at least 6
consecutive months from the date of disability.

Medical Expenses
The Medical Expenses incurred for surgical, clinical and
hospitalisation due to accidental causes directly related to the
participating sports that occurred during the coverage period, will be
reimbursed up to maximum of 10% of sum covered per accident.
The benefit will be payable if the event happens up to 14 days from
the date of accident.

This benefit can be claimed up to multiple times within the coverage


period, provided that next claim is caused by different accident Up to Up to Up to Up to

Payment under this benefit will not accelerate the sum covered of
this product.

Note: 1% of sum covered, capped at RM50 as deductible will be


applicable per accident, where the amount will be borne by the
Person Covered.

Note:
a) For initial launch, only plan 1 will be available; while plan 2 to plan 4 will be made available in future.
b) The coverage is valid for one (1) year only.
c) Sports and activities must be Shariah compliant, not professional sports, and do not constitute any gambling
activities. For the avoidance of doubt, “Professional Sports” means engaging in any physical activity in a
professional capacity or where the Person Covered would or could earn income or remuneration from
engaging in such activity.
d) Sports means an activity involving physical extension and skill in which an individual or team competes
another or others for entertainment or performed for leisure including participating in races or competition but
solely as amateur athletes excluding motorcycling and driving a motorcar whether it is a race or not.
e) Each Person Covered is allowed to participate in this certificate only once at a time. Multiple coverages or
plans on the Person Covered under this certificate is not allowed.

4. How much contribution do I have to pay?


The contribution payable depends on the plan selected by you. Please refer to the Schedule of Contribution as
sets out below:

Subject to renewal age eligibility, you may renew your cover annually by continue paying the contribution.

The contribution rate is not guaranteed and subject to review by Us. We reserve the right to revise the
contribution, benefit structure and renewal upon certificate renewal by giving the Person Covered 30 days
advance written notice. The revised contribution, if any, will aim to reflect actual claim experience or other
justified circumstances.

5. What are the fees and charges that I have to pay?


Upfront Charge is deducted from the Tabarru’ Fund to pay for distribution related expenses including
commission and management expenses. In addition, the contribution quoted in Question 4 above will be
subjected to additional processing fee. Please refer to the following table for details:

6. What are some of the key terms and conditions that I should be aware of?
 Importance of Disclosure – you must disclose all material facts such as medical condition and state your
age correctly.
 Free-Look Period - you may cancel the Certificate within 15 days of receipt of the notification from Us to
you, provided no claim has been admitted under your coverage. We shall refund an amount that equals to
the contribution paid.
 Grace Period - a grace period of 30 days from the contribution due date is given for you to make the
subsequent contribution.
 Claims Procedures - notification should be made immediately upon the occurrence of the accident or within
the time frame stipulated in the terms and conditions of the Master Certificate. Claims forms can be obtained
from Our website, where you must submit a duly completed claim form together with the necessary
documents. In this respect, the documents required may vary according to types of claims. Proof of claim
must be accepted and approved by Us to facilitate payment of any benefit.
 Eligibility – you must be a Malaysian or permanent residents in Malaysia, between 18 and 50 years age
next birthday. Entry age next birthday refers to the attained age next birthday of each Person Covered on the
Risk Commencement Date.
 Fitness for Sports – you must be medically fit to participate in the sport undertaken and must not participate
in the sport against doctor’s advice.
 Waiting Period – the eligibility for benefits under the Certificate will start on the next day after participating in
this Certificate.

Note: This list is non-exhaustive. Please refer to the Master Certificate for the terms and conditions under this
plan.

7. What are the limitations and major exclusions under this certificate?
Exclusion for Critical Illness
No benefit will be payable if the illness:
 has existed prior to or on the Effective Date;
 is caused directly or indirectly by a self-inflicted injury;
 is resulted from the Person Covered committing, attempting or provoking an assault or a felony from any
violation of law by Person Covered;
 is resulted from war, whether declared or undeclared;
 is caused directly or indirectly by the existence of Acquired Immune Deficiency Syndrome (AIDS) or by the
presence of any Human Immunodeficiency Virus (HIV) infection. We reserve the right to require the Person
Covered to undergo a blood test for HIV as a condition precedent to acceptance of any claim. The exception
is when HIV due to blood transfusion as defined in this Certificate. For the purpose of this Certificate,
infection shall be deemed to have occurred where blood or other relevant test(s) indicate in Our opinion
either the presence of any HIV or antibodies to such a virus;
 is diagnosed due, directly or indirectly, to a congenital defect or disease, which was manifested or was
diagnosed before the Person Covered attains 17 years of age;
 is diagnosed, and the Person Covered survives less than 7 days after the day of diagnosis;
 is caused by injury arising from Professional Sports and illegal activities. For the avoidance of doubt,
“Professional Sports” means engaging in any physical activity in a professional capacity or where the Person
Covered would or could earn income or remuneration from engaging in such activity;
 is caused by performing Sports whilst Person Covered does not hold a valid qualification or Person Covered
sustained bodily injury due to his/her failure to comply to the age, safety or fitness requirements which are
required by the related Sports qualification;
 is caused by performing Sports although the Person Covered is deemed as not fit to perform the Sports or
perform the Sports against doctor’s advice; or
 is caused by performing Sports which is deemed as Shariah non-compliant at Our sole and absolute
discretion.

Exclusion for Accidental Death/Accidental TPD/Medical Expenses


No benefit will be payable if caused by the following events:
 suicide, attempted suicide or self-inflicted injuries, while sane or insane;
 bodily infirmity, or mental or functional disorder, or illness or disease of any kind, or any infections, other than
infections occurring simultaneously with and in consequence of an accidental cut or wound;
 any Accident caused by mosquito bites, worm infestations such as “Hookworms” and allergic reaction to
insect bites;
 war or any act of war, declared or undeclared, criminal activities, active duty in any armed forces, direct
participation in strike, riots and civil commotion or insurrection;

 from the action of any armed forces, or from Accident or violence arising by reason of the existence of a
state of armed conflict;
 engaging in aerial flights other than as a crew member or as a fare-paying passenger of a licensed
commercial airline operating on a regular scheduled route;
 while under the influence of alcohol or drugs unless taken as prescribed by a Physician;
 is caused by injury arising from Professional Sports and illegal activities. For the avoidance of doubt,
“Professional Sports” means engaging in any physical activity in a professional capacity or where the Person
Covered would or could earn income or remuneration from engaging in such activity;
 is caused by performing Sports whilst Person Covered does not hold a valid qualification or Person Covered
sustained bodily injury due to his/her failure to comply to the age, safety or fitness requirements which are
required by the related Sports qualification;
 is caused by performing Sports although the Person Covered is deemed as not fit to perform the Sports or
perform the Sports against doctor’s advice; or
 is caused by performing Sports which is deemed as Shariah non-compliant at Our sole and absolute
discretion.

Note: This list is non-exhaustive. Please refer to the Master Certificate for the full list of limitations and
exclusions under this plan.

8. Can I cancel my Certificate?


You may email directly at GreatAssist@greateasterntakaful.com for the cancellation request of your coverage at
any time. However, you will not be entitled to a refund of contribution for cancellation after the 15 days free-look
period. Despite the cancellation, the coverage will still continue until the end of the next contribution due date.

9. What do I need to do if there are changes to my/my nominee(s) contact details?


It is important that you inform Us of any change in your/your nominee(s) contact details to ensure all
correspondences reach you/your nominee(s) in a timely manner.
