    AFFIDAVIT OF LOSS

    I, DEBORAH BANZON MORABE, of legal age, married, residing at Gun-ob, Lapu-Lapu


    City, Philippines, after being sworn in accordance with law, depose and say:

    1. That I am the mother of JOSE ANDRE B. MORABE who is the owner of Persons
    With Disablity (PWD) Identification card issued by Persons with Disability Affairs
    Office (PDAO);

    2. That I usually kept the PWD ID of JOSE ANDRE B. MORABE inside my wallet for
    safe keeping;

    3. That sometime on March 2022, after arriving home, I noticed that my wallet is
    already missing from my pocket wherein the abovementioned item was kept and
    it might have fallen out of my pocket while I was travelling;

    4. That I directly went back to my route and diligently exerted efforts to locate the
    said PWD ID, but despite a diligent search it could not be found, hence the same is
    deemed lost and at a point beyond recovery;

    5. That I am executing this affidavit to attest to the truth and veracity of the
    foregoing facts so that I may be issued a new Persons WithDisability (PWD)
    Identification card in lieu of the lost one, and for whatever legal purpose this may
    serve.

    IN WITNESS WHEREFORE, We have hereunto affixed our signature this 25th day of
    August, 2022, at Cebu City, Philippines.
