Dear participants,

Greetings of Peace!
The undersigned are fourth-year students taking Bachelor of Public Administration (BPA) at
Southern Luzon State University – Main Campus (SLSU) and currently working on their RES02
– Research Method in PA 2 as part of their undergraduate requirement.
In line with this, the research study entitled "BIYAHENG POGI: IMPLEMENTATION OF
SAFETY REGULATION IN MARITIME TRANSPORTATION IN SELECTED
MUNICIPALITY IN QUEZON PROVINCE" will determine the perceived level of
implementation of safety regulation among the MARINA, coast guard, operators, barangay
officials/member of Sangguniang Barangay, crews, and passengers.
This questionnaire will ask you to answer two-part questionnaire based on the objectives. Part I is
to group the respondents as to MARINA, coast guard, operators, barangay officials/member of
Sangguniang Barangay, crews, and passengers while the part II will be level of implementation
safety regulation in terms of life-saving equipment, fire protection, and operational requirement.
Rest assured that all information collected will remain confidential and that the only foreseeable
harm is the invasion of privacy, as sharing of personal experiences regarding school matters is of
utmost needed for the completion and accomplishment of the research study.

Our sincere gratitude for your time and cooperation in this matter.

Respectfully Yours,
