Home and Environment

Mr. Nico and Mrs. Ma. Gerlyn have been living with Mr. Nico’s parents since they were married. Mr.
Nico’s parents own the house located at Zone 7, Brgy. San Isidro, Magarao, Camarines Sur.

The house is semi concrete and has two rooms. The first room is for Mr. Elmer, Mrs. Ma. Teresa, and Ms.
Marisol, the second room is for Mr. Nico together with his wife and baby. The location of their residence
is on a well ventilated area as it is nearby the rice field so the air is fresh, but, inside their house, it has
poor ventilation as it only consist of limited windows and isn’t spacious enough to have a proper air
circulation.

The Medenilla family has only five types of appliances, including a small refrigerator, a television, a
washing machine, a kitchen stove, and an electric fan. In terms of waste disposal, the community don’t
have a garbage collector so they place their trash in a sack and burn it in their backyard.

Mrs. Ma. Teresa is the one who cooks and prepares food most of the time but Mr. Elmer do cook also
when he is not at work. Their cooking utensils are complete and they use stainless spoon and fork,
plastic plates, and ordinary glasses when eating.

The source of water of the family is water well, they use it to take a bath and wash dishes and clothes.
The family’s drinking source is mineral water, which delivered to their home once in a while.

The family is near the hazards because there is a dirty creek and open drainage near their house. The
area of their residence is not safe from flooding because the house is on low ground, and there are
limited trees surround it. The drainage system is open and located at the back of their house.

The family does not have any domestic animals. They own one vehicle and their primary transportation
are jeep and motorcycle. They use their automobile to go to work, social, and health facilities.

The nearest health facility for the family is the barangay health center. In terms of food resources, there
is no market near them so they travel outside their barangay to buy fresh foods such vegetables, fruits,
meat, and fish.

D. Health Assessment of Each Family Member

D1. PAST AND PRESENT ILLNESSES

a. Mr. Elmer Medenilla – during the interview with the family, Mrs. Ma. Teresa said that Mr. Elmer
does not have any past illness. He has a bad habit which is addicted to cigarettes. So, there is a
possibility that he can develop a cancer, particularly lung cancer. He doesn’t drink alcohol even
when there is an occasion. He is not COVID 19 vaccinated.
b. Mrs. Ma. Teresa Medenilla – She is diabetic and been hospitalized before. She is taking
medication now which is glipizide that helps lowers her blood sugar levels. She sometimes take
herbal medicine, the serpentina which is also good for diabetic person. Mrs. Ma. Teresa has
done an operation also before because of her ‘pigsang dapa’ on year 2021 that makes her
oblivious. She doesn’t have any bad habits and completed 1st and 2nd dose of COVID 19 vaccine.
c. Ms. Marisol Medenilla – Mrs. Ma. Teresa said that her daughter does no have any past and
present illness. She only had a chicken pox before and cough and colds lately. She doesn’t have
any bad habits such smoking and she’s COVID 19 vaccinated.
d. Mr. Nico Medenilla – Just like Ms. Marisol, Mrs. Ma. Teresa said that her son does not have any
past illness. He doesn’t smoke but drinks alcohol only when there is an occasion. He is COVID 19
vaccinated with no present illness.
e. Mrs. Ma. Gerlyn Medenilla – She is COVID 19 vaccinated with no past and present illness. She is
taking food supplement and soup with malunggay as a home remedy because after giving birth
to her first child, there is no milk coming out on her breasts. She doesn’t have any bad habit.
f. Child Philan Caleb Medenilla – As of today, he has no chronic or congenital disease. He is living
healthy and have done BCG and HEPA B vaccines. Other vaccines are to be followed as he is only
two weeks old as of now.

D2. FAMILY HEALTH ASSESSMENT BASED ON FUNCTIONAL HEALTH PATTERN

a. Health Perception – Health Management Patterns


 Mrs. Ma. Teresa drinks Serpentina Herbal Juice for her high blood sugar levels.
 Mr. Elmer smokes but doesn’t drink alcohol even when there is an occasion.
 Mr. Nico drinks alcohol only when there is an important occasion.
 If someone has a fever in a family member, they give paracetamol or bioflu.
 The family has been able to recognize the importance of healthy well-being.
b. Nutritional-metabolic Pattern
 Mrs. Ma. Teresa is taking B-complex vitamins.
 The whole family has good appetite.
 Rice, fish, fruits, and vegetables are their common daily food intake. They only eat meat
and processed foods sometimes.
 The daily fluid intake is more than 8 glasses of water.
c. Elimination Pattern
 There is no problem with their urine.
 Eliminates standard pee recurrence regularly: 4 times a day
d. Activity-exercise Pattern
 No one in the family members does exercise because of their busy schedule at work.
 Their spare time activities are watching television and YouTube.
e. Sleep-rest Pattern
 There is no difficulty in sleeping in any member of the household, verbalize by Mrs. Ma.
Teresa.
 Mrs. Ma. Teresa said that they usually sleep around 11:00 pm and wake up at 6:00 am.
 The family sometimes doesn’t complete an eight-hour sleep due to work but Mrs. Ma.
Teresa stated that she usually doesn’t complete an eight-hour sleep because of her
grandchildren who sometimes visited them on their house together with their parents.
f. Cognitive-perceptual Pattern
 A family member has a sensory defect, particularly eyesight.
 As the respondents, Mrs. Ma. Teresa were able to respond to the questions
appropriately and accurately.
 The memory is intact.
g. Self-perception/Self-concept
 Mrs. Ma. Teresa has indicated that the things that makes her easily mad are her children
who sometimes doesn’t follow her order. Also, his husband when he smokes every single
hour.
h. Roles and Relationship
 All members of the family are open to discuss their problem with each other, said Mrs.
Ma. Teresa.
 They are not currently facing any problems.
i. Sexual Reproductive
 Mr. Elmer and Mrs. Ma. Teresa are already not reproductive and are not active in sex.
 The son, Mr. Nico and his wife are still reproductive and are active in sex.
j. Coping Stress
 They spend time together, share chores, keep communication channels open, and sit
down for an evening meal together several times a week to cope with stress.
k. Values/Belief Pattern
 The family are all Roman Catholic.
 They go to church every Sunday.
 They have values and beliefs in their religion and also believes in a quack doctor and a
fake healer.
l. Values, Habits, Practices on Health Promotion, Maintenance, and Disease Prevention
 The family members are all COVID 19 vaccinated except Mr. Elmer.
 They have healthy lifestyle practices such adequacy of rest, use of protective measure,
and other activities.
 No one in the family does exercise because they do not have time due to their job and
household duties.
 Mrs. Ma. Teresa stated that maintaining the health of her family is essential, that is why
they follow preventive measures.
 When one of the family members get sick, they sometimes self-medicate, but they also
make use of the health facilities in their barangay.
 The family believe in both herbal remedies, fake healers, and drug prescribed over the
counter as a means to cure their illness.

Chapter IV

FAMILY BACKGROUND

A.Family History

Family Medenilla is composed of six members wherein Mr. Elmer, the father, who is 51 years old, and
Mrs. Ma. Teresa, the mother, who is 52 years old. Jennifer, Marisol, and Nico are their children. Jennifer
is 28 years old, Marisol is 26 years old, and Nico is 24 years old. Ma. Gerlyn is the wife of Mr. Nico who is
also 24 years old, and Philan Caleb, their son, who is a neonate.

Mr. Elmer were born and grew up at San Pantaleon


Mrs. Ma. Teresa were born at Bicol Medical Center and grew up at San Isidro, Magarao.

Mr. Elmer and Mrs. Ma. Teresa met in their work. They became a couple on year 1990 and got married
on year 1994, they are married for 29 years now. They resided at Zone 7, Brgy. San Isidro, Magarao,
Camarines Sur since year 2002 up until now.
