With Aviva it’s reassuring to know that you’ll have access to the medical care
you need when you need it most.
This booklet details the various hospital options available that offer a comprehensive range of treatments. At Aviva we have two routes
to hospital treatment – Aviva Quality Guiding and Aviva Hospital Lists. If you refer to your policy documents, they will tell you whether
you have Expert Select, Optimum Referral or one of our Hospital List options.
Expert Select and Optimum Referral
If you have one of our Quality Guiding hospital options (Expert Select or Optimum Referral), it couldn’t be easier. Just ask your GP for
an open referral (one that specifies the type of specialist or specialism you need, but not a named specialist), and then call the customer
service helpline. Our claims experts will provide you with a choice of local hospitals/facilities and specialists that you can use, and can
book your first appointment for you there and then.
Hospital Lists – Extended List, Key List, Trust List, Signature List, Fair & Square List, Speedy Diagnostics List.
If you have a Hospital List option, you should always telephone our customer service helpline prior to receiving any treatment to
confirm that the hospital you’ve selected is recognised by us to provide the type of treatment you require and for the condition that
requires treatment. Hospitals available on your hospital list are colour coded on the Hospital List Summary, below.
Please check your policy documentation to see the hospital list that applies to your policy.
Please be aware that out-patient CT, MRI and PET scans will only be covered at a diagnostic centre approved by Aviva. Please call
customer service helpline to check which diagnostic centres are Aviva approved before having these investigations.

Quality Guiding Options – Expert Select & Optimum Referral

Expert Select and Optimum Referral are our two Quality Guiding hospital options. Expert Select is available for individual customers and
companies of 1-249 employees. Optimum Referral is available to companies with 250 employees or more. When you have one of our Quality
Guiding options, there’s no Hospital List. We provide you with a choice of quality assured hospitals (4-5 hospitals UK avg.), local to you (within
25 miles), when you need medical care. Hospital choices available to you will depend on your personal diagnostic or treatment needs at the time
you make a claim, so the facilities available to you will vary. All major private hospital groups are included on Expert Select and Optimum Referral.
Quality Guiding provides simpler access to treatment and the support of our expertise to guide you to quality assured hospitals and
facilities in your area. On Expert Select and Optimum Referral, we prioritise those hospitals who meet specific quality standards and
who strive to improve customer and patient experience.
If you have Expert Select or Optimum Referral, we will apply our quality guiding to both your out-patient and inpatient care.
If you are a customer with Expert Select or Optimum Referral, the Hospital List Summary in this document does not apply.

Networks Hospital List Options

Networks provide even greater quality assurance – quality guiding
for specific condition types. Using clinically selected Networks Key Hospital List

Signature Hospital List

This list is available for customers living in Scotland or
Northern Ireland.
In addition to these hospitals you are also covered to use NHS
pay-beds that are recognised by us in Scotland or Northern Ireland.

If you have in-patient or day-patient treatment at any other hospital

recognised by us, we’ll calculate the average cost of equivalent
treatment across all hospitals on your list, and that average cost is
the maximum we’ll pay. This could leave you with a shortfall that the
policy doesn’t cover. If the actual cost of the treatment is less than
the average cost, we’ll pay the hospital costs in full.

If you receive treatment in a hospital that isn’t recognised by

us, we won’t pay any hospital fees for your treatment.
