1. Please select 2 CLASS REPRESENTATIVES (Leader & Assistant Leader) for this First
Semester. They will act as facilitators for the entire class for any concerns and
information dissemination.
2. As a class, make a Facebook and MS Teams group chat as well for the announcements
and specific instructions. Please refrain from entering information or posts not essential
or not related to the subject so that we will prioritize essential data for everybody’s
usage and reference on a real time basis as much as possible.
3. CLASS SCHEDULE will be the following:

4. Every session there will be an assigned CLASS MONITOR to check the attendance for the
entire session (Please have it in ALPHABETICAL ORDER for fair chance). Appropriate
template form is provided by the clinical instructor for the said subject. Please use only
BLACK or BLUE BALLPEN for the entry of the attendance sheet.
5. For each student, please always bring the following during class session for proper
attendance and documentation of participation:
- Daily ORANGE long size folder to be fastened to all the needed forms for the said
subject.
(CI in charge will give the prescribed forms for strict compliance)
- BLACK and RED ballpens
6. Please wear appropriate school lecture uniforms when presenting oneself in the
classroom session.
7. For instance, if a student shall be absent for a reason, he/she should present an
explanation letter (format be given in a while) and a medical certificate (if applicable)
that is translated in the university school clinic for it to be accepted and valid.
8. Always maintain camaraderie and professional courtesy during the entire session. For
your personal queries and concerns, please address it directly to the clinical instructor
soonest time.
9. Please always adhere to the stipulated schedule unless the clinical instructor announces
prior for the changes of schedule. Each student will be assigned a certain topic to
present for the entire summer.
12. Please take note of the PASSING PERCENTAGE, that is 65%. Please always compute your
score examinations to monitor your progress. Midterm and final performances
consultation will be conducted on a one-on-one (face to face) appointment between the
clinical instructor & each student.
13. Erasures/ alterations/superimpositions in any data/ quizzes, long exams, term exams are
not allowed
14. Cheating in any form is not permitted and is punishable by expulsion.
