    INCIDENT REPORT ON DAMAGED COMPUTER UNIT UPON DELIVERY

    Dear Sir/ Madam,

    I would like to report in this good office regarding a damaged computer unit upon
    delivery that occurred on September 20, 2022, at _______ address.

    The consignor of the package is my employer from Australia. Upon opening of the
    said delivered package, I noticed that the item has been damaged. The damaged unit is a
    one (1) computer unit with a brand ____( model, specs).

    Attached herewith are the images of the said damaged computer unit taken upon
    delivery and the photo copy of the official receipt of delivery.

    This incident report is made for the purpose of an Insurance claim and for whatever
    purposes it may serve under the Philippine Law.

    Should you need further information about the incident, I would be open for
    discussion.
