1. Purpose
The project closeout report is developed and shared with all concerned parties and is relevant to swimming pool
renovation project.
The sole purpose of this report is to provide a summary of the products delivered, comparison of the baseline projects
as well as demonstrate the lessons learnt, commitment of the organization, HSE related issues raised during the
execution of the project and how they were addressed including the Project HSE Objectives. This Project Closeout
serves as the official closure of the project and provides a permanent record for reference for future project team.

2. Scope
The Project Closeout Report covers the Architecture, civil, engineering, procurement, materials management, site
management, and other activities with respect to the HSE.
Midas Real Estate Limited has accomplished all the activities since December 11th , 2023.

3. Team Members

4. Organization Management System


Organization developed an Integrated Management System comprising of following 3 standards listed below;
1) ISO 9001:2015 Quality Management System
2) ISO 14001:2015 Environmental Management System
3) ISO 45001:2018 Occupational Health and Safety Management System
4) Compliance with all the national and international legal requirements, code of practices, and client
requirements.

The organization has declared its policies that are reviewed and signed by the CEO/MD and HoD. The following policies

7. Quality, Health, Safety & Environment Objectives


Organization declared its objectives to be achieved for 2024 and those are also applicable to the Project. The project
team is bound to achieve these objectives by all means and organization is liable by law to provide resources and
budget to achieve these objectives. The objectives are listed below;
- Training and development of all the employees
- 3rd party training of all the employees
- Zero fatality at the workplace
- Zero incident and accident at the work place
- Zero environmental incident at the workplace
- Customer satisfaction by delivery of good quality product
- Workers training to enhance their awareness about environmental aspects/impacts
- Minimize exposure to dust
- Waste management improvement by implementing segregation, safe storage and handling, and transporting
techniques
- Zero Spillage at workplace

8. Project QHSE Achievements and Awards


8.1. Total Man Hours Achieved – Achievement #1
During the project, a team of 15 was hired to perform the job. The project started on 11th December, 2023 and came
to end on [Month Day, Year]. The total duration of the project is [Number of Days].

Total Accumulative Man Hours = 5,354


These total man hours are without any incident and this is how the objectives of Zero Fatality and Zero
Accident/Incident were achieved.

8.2. Accident & Incidents Statistics – Achievement #2


During the whole project no major accident/ incident took place neither any life was lost during the execution of the
job. Following statistics are measured and zero accident/incident objective was achieved.
8.1.1. TRIR – Total Recordable Incident Rate
TRIR = 0 (Zero)
The Recordable Incidents are following;

Zero Spillage at workplace – Achievement


During the whole project the chemical spillage incident didn’t take place at all. All the workers were trained for
handling and storage of chemicals.

8.3. Worker Appreciation by Client – Achievement/ Award


For working safely and demonstrating excellent commitment towards health and safety, two workers were
appreciated. The both workers helped in implementing the positive health and safety culture at workplace.

8.4. Training and Induction of all Employees


All the employees were trained and inducted before start of the job.

8.5. Legal Notices and Violations


During the execution of the project there was no violation of the legal requirements neither any legal notice was
served.

9.1. Important Definitions


9.1.1. Fatalities
An employee’s death during work time due to any work-related incident or exposure to hazardous element.
9.1.2. Lost Time Injuries - LTIs
Lost Time Injury refers to the incidents that result in disability or an employee missing work due to injury.
9.1.3. Work Days Lost
Days away from work dur to an accident/incident that restricted the worker to return to work.
9.1.4. Work Restricted Days Case - WRDCs
If due to an accident, the worker is performing or is not able to perform his all routine duties he is supposed to do,
such incident will be addressed as Work Restricted Days Case.
9.1.5. Medical Treatment Cases – MTC
Any incident that lead to injury/ disease/ disorder that require a proper medical treatment instead of first aid.

9.1.6. First Aid Cases - FAC


Any incident that lead to minor injury and can be addressed by the onsite doctor/paramedic and require only first aid.
9.1.7. Material Loss
Any incident in which the product/material is lost due to any incident at the workplace.
9.1.8. Occupational Illness
Deterioration of existing health condition due to exposure to different agents e.g., biological agents, physical elements,
and mechanical trauma.
9.1.9. High Potential Incidents – HiPo
High potential incidents are the incidents and near-misses that can result in one or more fatality at the workplace
under extraordinary conditions.
9.1.10. Near Miss Incident
Any unplanned and unwanted incident that didn’t result in injury, death or loss but had the potential to do so.
