BRIEF HISTORY OF CAN-AVID NATIONAL HIGH SCHOOL

kilometer away from the own proper north bound covers a vast land area of about three and

forty-three hectares (3.43 has) stretching along the sea cost facing the Pacific Ocean. The

school site was acquired a deed of donation by the Local Government Unit (LGU). It is

situated along the national road that can be reached within ten minutes walking and/ or five

minutes ride through bicycle or estimated 3 minutes in a motor cab/ tricycle.

The secondary school in the Municipal proper was formerly located in Can-avid

National Agricultural College (CNAC) currently known as Eastern Samar State University

(ESSU) Can-avid Campus. It was in the year 2002 that the secondary was separated from

CNAC to what is now known as Can-avid National High School (CNHS) strategically located

along National Road in brgy. 4 Poblacion, Can-avid Eastern Samar. In addition, the school

started to operate way back October 23, 2002 as a newly independent secondary school

governed by Eastern Samar Division of the Department of Education (DepEd).

The school has a wide vicinity ground. It has currently 9 two-story building, and 1 was

an old two-story building that was built through the comprehensive development fund of

Senator Joker Aroyo worth 5.5 million. Also, it has 10 one-story building which serves as an

academic building, 3 TVL building, 1 Gymnasium, 2 buildings That serves as school Admin,

a canteen, hand washing area and a variety of comfort rooms that includes LGBT comfort

rooms.

The school has a perimeter fence guarding the entire vicinity of the school and has a

newly constructed Guard post located besides the e-classroom building.

As of this moment, CNHS is SBM level III School and it operates senior high school

strands with a newly opened strand Science Technology Engineering and Mathematics

(STEM).

III. NARRATIVE REPORT

BEFORE DEPLOYMENT

Before we had our On the Job Training as Students teachers’ trainee in our

respective Cooperating School we had our Pre-Demonstration teaching at Eastern Samar

State University Can-avid Campus. It was the way to measure if the Students teachers are

ready for the field of practice. In addition, we also attended series of activities like seminars

and doing our room structuring.

DURING DEPLOYMENT

On the Job training is a practical approach to acquiring new competencies and skills

needed for a job in a real, or close to real working environment. It is often used to learn how

to use particular tools or equipment in live-work practice, simulated, or training environment.

This training happens at the workplace, with guidance from supervisors, heads, managers,

or other knowledgeable employee.

April 18, 2022 when we had the Deployment of students’ teachers to the respective

Cooperating school, it was also the day we arrived at Can-avid National High School. After

we arrived at CNHS we were welcome by the CNHS school staff together with their principal.

After that, we had our Students’ teacher’s orientation with the AP coordinator and we were

assigned to our respective Cooperating Teachers.

April 19, 2022 the second day with the Cooperating school, We Were ask to attend

the final instruction of our Students’ teacher coordinator at the Science Lab. The coordinator

told us what things that we are expected to do and to attend too. It was also the first day of

our observation with our CT in an actual class discussion.

April 20&21, 2022 was just the same observations I have with my Cooperating

teacher in actual class discussion.

April 22, 2022 my Cooperating teacher told me to conduct an ESP activity to his

students at Grade 9 section Cottonwood because he was busy at that time. After that I have

my observation again on the rest of the day.

April 23, 2022 it was Saturday, I attended their 3rd quarter portfolio day at Grade 9

section sycamore. I observed the actual collaboration of teachers and the parents of

students.

April 25 to 29 it was the first week of my actual Pre-demonstration teaching for the

four section and two subjects, set (B) students was my first students.

May 4 & 6, 2022 we ask to help our Student teachers Coordinator to arrange the pre-

registration of upcoming grade 11 students of Can-avid National High school. It was a two

day activity while the whole school is preparing for the National and Local Election.

May 16 to 20, 2022 it was the second week of my actual Pre-demonstration teaching

for the four section and two subjects, this time it was the set (A) students was my second set

of students.

May 23 to 27, 2022 it was the third week of my actual Pre-demonstration teaching for

the four section and two subjects, this time it was the set (B) students again. In addition, May

27, Friday our students’ teacher coordinator calls for a meeting for the preparation of our

final demonstration.

May 30 & 31, 2022 it was the fourth week of my actual Pre-demonstration teaching

for the four section and two subjects, this time it was the set (A) students again. It was

followed by June 1-3, 2022 to complete the five day class discussion. In line with this June

3, 2022 we were ask to attend the First Friday mass of Can-avid National High School.

June 6 & 7, 2022 last two days of actual pre-demonstration teaching for the set (B)

set of students. After that, it was followed with the preparation for my final demonstration.

June 8 & 9, 2022 was the two days preparation for the final demonstration teaching

to be held at Grade 9 section Sycamore. It was followed by June 10, 2022 the day for my

final demonstration. In line with this a post conference was also conducted after the

deliberation of my final demonstration.

June 13 & 14, 2022 it was the 4th quarter parallel examination. I conducted the

examination to the four section composing two sets which is set (A) and set (B).

June 15 to 16, 2022 just after the examination I was ask by my cooperating teacher

to compute the 4th quarter grades of my students and thankfully it was finished immediately

INTERNSHIP AGREEMENT
(Host Company/Institution and Student Intern re: Internship)

This Memorandum of Agreement (hereinafter “Agreement”) is made and entered into


on APRIL 2022 in Can-avid, Eastern Samar, Philippines by and between:

CAN-AVID NATIONAL HIGH SCHOOL a Company duly organized and


existing under and by virtue of the laws of the Republic of the Philippines with
address at BRGY. 04, CAN-AVID, EASTERN SAMAR (6817) duly
represented by its SCHOOL PRINCIPAL IV, ROSANNA G. CATUDAY and
hereinafter referred to as (CAN-AVID NATIONAL HIGH SCHOOL);
-and-
KIM T. CATUDAY a Bachelor in Secondary Education Major In Mathematics
student of the College, of Education, EASTERN SAMAR STATE UNIVERSITY
(ESSU), of legal age, with address at Brgy. 08, Can-avid, Eastern Samar ,
hereinafter referred to as “Student”.

______Witnesseth—

WHEREAS, ESSU instituted an internship program as part of the curriculum for its
(degree) students in order to provide them with adequate experience in actual industry work
operations and to augment their formal education;

WHEREAS, CAN-AVID NATIONAL HIGH SCHOOL, in its desire to enhance the


training and development of more and more professionals, has agreed to provide support to
the students of ESSU in the form of training on the operations and latest developments in its
industry; and

WHEREAS, ESSU and the CAN-AVID NATIONAL HIGH SCHOOL executed a


Memorandum of Agreement that covers the internship program’s objectives and principles
pursuant to their established Internship Plan for KIM T. CATUDAY.

WHEREAS, I, KIM T. CATUDAY accepts this Agreement and the respective


Internship Plan with all its terms and conditions as contained herein.

NOW, THEREFORE, for and in consideration of the foregoing premises, the Parties
hereby agree as follows:

SECTION I
Responsibilities of the Company/Institution

a) CAN-AVID NATIONAL HIGH SCHOOL shall provide free relevant instruction, exposure,
and training to KIM T. CATUDAY, consistent with its policies, rules and regulations and
with the established Internship Plan. It shall treat KIM T. CATUDAY in a professional
manner.

b) CAN-AVID NATIONAL HIGH SCHOOL shall ensure that, KIM T. CATUDAY in the
course of training, shall not be exposed to any form of harassment/unethical practice or
tasks and work assignments that are unreasonably risky, dangerous or unrelated to the
purposes of the Internship Plan;

SECTION II

Responsibilities of the Student

a) I, KIM T. CATUDAY shall abide by the administrative policies, rules, and standards of
CAN-AVID NATIONAL HIGH SCHOOL and ESSU;

b) I, KIM T. CATUDAY shall accomplish and submit to the assigned ESSU Internship
Coordinator a weekly journal of activities, to include success stories as well as
challenges and reflections on the training experience;

c) I, KIM T. CATUDAY shall report to the ESSU Internship Coordinator within two (2) days
after completion of the training;

d) I, KIM T. CATUDAY shall accomplish the assigned tasks to the best of his/her abilities
and conform with the Internship Plan set forth by ESSU and the Company/Institution;

e) I, KIM T. CATUDAY shall complete the agreed scope of work within the agreed duration
of the Program; and

f) I, KIM T. CATUDAY shall timely report for training and promptly accomplish assigned
tasks or assignments in the best of his/her ability and in view of his/her expected skill
and knowledge of the subject matter.

SECTION III
General Conditions

a) The duration of the program shall be equivalent to Three hundred Sixty (360) working
hours commencing on April 2022 to June 2022 unless otherwise agreed upon by the
Parties herein;

b) I, KIM T. CATUDAY shall report for work at CAN-AVID NATIONAL HIGH SCHOOL
(venue of office/facility/fieldwork area) located at (address of work area/office) from (start
time) to (end time), which is inclusive of a one (1) hour break/lunch time and reasonable
morning/afternoon break-times not exceeding fifteen (15) minutes;

c) CAN-AVID NATIONAL HIGH SCHOOL and ESSU jointly developed an Internship Plan
for KIM T. CATUDAY that specifying objectives, knowledge, skills, and competencies
that KIM T. CATUDAY should acquire in each learning area, assignment, and/or activity
covered by the Internship Plan attached herewith as Annex “A”;

d) CAN-AVID NATIONAL HIGH SCHOOL MAY provide an honorarium to the student


subject to the policy on student-allowance and/or reimbursement of the CAN-AVID
NATIONAL HIGH SCHOOL.

e) CAN-AVID NATIONAL HIGH SCHOOL and ESSU will conduct a post-training review
and evaluation of the program and the performance of KIM T. CATUDAY

f) CAN-AVID NATIONAL HIGH SCHOOL is not obliged to employ KIM T. CATUDAY


upon completion of the training. CAN-AVID NATIONAL HIGH SCHOOL, however, upon
consultation with the ESSU, may invite qualified students to submit themselves to
examinations, interviews, and file pertinent documents in support of their application;

g) I, KIM T. CATUDAY shall be personally responsible for any and all liabilities arising from
his/her gross negligence in the performance of his/her duties and functions while under
training with CAN-AVID NATIONAL HIGH SCHOOL;

h) The Parties shall not divulge any information that it may have access to and any such
information will only be used for academic purposes;

i) This Agreement shall not, in any way, constitute an employee-employer relationship


between CAN-AVID NATIONAL HIGH SCHOOL with KIM T. CATUDAY

j) I, KIM T. CATUDAY, shall be personally responsible for any and all liabilities, injury to
his/herself or to property or persons attributable to his/her own fault or negligence while
in the course of his/her training. In this light, I shall hold CAN-AVID NATIONAL HIGH
SCHOOL or ESSU free and harmless from any demand, claim or complaint, whatsoever
arising from this training agreement, except in cases of gross negligence, malicious acts
and criminal acts by any of the officers, employees or agents of CAN-AVID NATIONAL
HIGH SCHOOL or ESSU;

k) In the event that KIM T. CATUDAY materially violates any company policy, the CAN-
AVID NATIONAL HIGH SCHOOL shall have the right to immediately, with due process,
terminate the Internship Plan in relation to such student by immediately serving a written
notice to ESSU;

SECTION IV
Internship Plan

a) I, KIM T. CATUDAY is expected to perform office/work-related tasks such as, but not
limited to, __________________________ applicable pursuant to the established
Internship Plan;

b) I, KIM T. CATUDAY, shall perform other tasks or responsibilities as reasonably directed


by CAN-AVID NATIONAL HIGH SCHOOL in relation to the respective Internship Plan
established by ESSU and CAN-AVID NATIONAL HIGH SCHOOL; and

c) The Internship Plan shall be deemed as an integral part of this Agreement.

SECTION V
Intellectual Property

a) Any intellectual property owned by the Parties prior to this Agreement shall continue to
be owned by them. KIM T. CATUDAY cannot use any confidential information or data
from CAN-AVID NATIONAL HIGH SCHOOL to create intellectual property without the
express written approval of CAN-AVID NATIONAL HIGH SCHOOL;

b) Any intellectual property independently created by KIM T. CATUDAY during the period
covered by the internship plan shall be exclusively owned by KIM T. CATUDAY. Should
CAN-AVID NATIONAL HIGH SCHOOL provide financial support for the creation of
such intellectual property, the ownership of the intellectual property shall be subject to
another agreement;

c) In case KIM T. CATUDAY jointly creates intellectual property with a person connected
with CAN-AVID NATIONAL HIGH SCHOOL, the intellectual property shall belong to
them jointly in proportion to their contribution.

SECTION VI

Confidentiality and Waiver

a) It is expressly understood by KIM T. CATUDAY that all information on technology,


manufacturing process, process standards, quality assurance methodologies, quality
standards, production capabilities, raw materials purchasing, marketing, finance, and all
other related documents, manuals, operational or technical matters that CAN-AVID
NATIONAL HIGH SCHOOL shall make available shall be used for the sole purpose of
performing their tasks under the internship program.

b) It is understood that all information gathered by KIM T. CATUDAY on the operations and
business matters of CAN-AVID NATIONAL HIGH SCHOOL are classified as
confidential in nature and proprietary to CAN-AVID NATIONAL HIGH SCHOOL. KIM T.
CATUDAY hereby undertakes to prevent transfer of such information by any of its
members, consciously or unconsciously, to any party outside of CAN-AVID NATIONAL
HIGH SCHOOL without the latter’s knowledge and written consent.

c) During, or at any time after the termination of Internship, the Intern shall not use, or
disclose to others, including future employees, any trade secrets, confidential
information, or any other proprietary data of CAN-AVID NATIONAL HIGH SCHOOL;
and

d) That upon the termination of Internship from CAN-AVID NATIONAL HIGH SCHOOL,
KIM T. CATUDAY shall return to CAN-AVID NATIONAL HIGH SCHOOL all documents
and property of CAN-AVID NATIONAL HIGH SCHOOL obtained during the course of
Internship; and

e) KIM T. CATUDAY renounces and waives any claim against CAN-AVID NATIONAL
HIGH SCHOOL or ESSU for any injury or loss that KIM T. CATUDAY may sustain or
may suffer, personal or pecuniary, in the performance of his/her duties and functions
while under training, unless such is attributable to the negligence or fault of any of the
employees, officers, or personnel of CAN-AVID NATIONAL HIGH SCHOOL or ESSU.

SECTION VII
Termination of the Agreement

The Parties shall have the right to pre-terminate the training contemplated by this
Agreement and the Internship Plan by serving a written notice to the other party at least
thirty (30) days prior to the intended date of termination and based on the following grounds:

a) KIM T. CATUDAY substantially violates the rules and regulations of CAN-AVID


NATIONAL HIGH SCHOOL, or in cases of serious misconduct of KIM T.
CATUDAY;

b) CAN-AVID NATIONAL HIGH SCHOOL is proven to fail in providing the safety of


KIM T. CATUDAY and quality of training contemplated by this Agreement and
the established Internship Plan;

c) There is any material violation of the foregoing covenants that will warrant the
cancellation of this Agreement.

SECTION VIII
Entire Agreement and Amendment

This Agreement constitutes the entire agreement between the Parties with respect to
the subject matter hereof. It excludes and supersedes everything else which has occurred
between the parties whether written or oral, including all other communication with respect to
the subject matter hereof. Any amendment hereto shall be mutually agreed upon in writing
by the parties.

SECTION IX
Dispute Resolution, Governing Law, and Venue of Actions

Any and all disputes arising out of or relating to this Agreement shall be subjected to
good faith negotiations between the Parties before implementation of the legal proceedings
pursuant to the provisions of the Alternative Dispute Resolution Law.

This Agreement shall be governed by and construed in accordance with the laws of
the Republic of the Philippines. Exclusive jurisdiction over and venue of any suit arising out
of or relating to this Agreement will be in the proper courts.

IN WITNESS WHEREOF, the parties have signed this Memorandum of Agreement


at the date and place above stated.

Nowadays, the

market is very much

demanding and we often

come across the issue

regarding the level of higher

education meeting the

industry expectation. To

bridge the gap and equip

the students of current

generation with new-age of

technologies, seminars and

workshops play an

imperative role. Ensuring a proper flow of knowledge Seminars and workshops assist in

passionate interaction and active participation boosting the skills and expertise of students.

Before the deployment of the students’ teachers, we attended different seminars.

Two webinars, one from ESSU-Guiuan entitled “IATF Guidelines and Covid-19 health

protocols for student teachers” and one from ESSU-Borongan which is entilted “HAG-IICD A

pre-deployment seminar for student interns” and two seminars inside the campus of ESSU-

Can-avid. One was in titled “Seminar on anti-bullying, child protection act, and safe school

environment. And lastly, the send-off seminar for the students teachers.
