PREFACE
It gives me an immense pleasure to publish the report of Hospital Based Cancer

registry (HBCR) for the year 2015.
The Hospital Based Cancer Registry has been functioning since long as per the
standards and norms prescribed by the National Cancer Registry Programme
(NCRP) of the Indian Council of Medical Research (ICMR).
From January 2014, the National Cancer Registry Programme (NCRP)-National

Centre for Disease Informatics and Research (NCDIR) - Indian Council of Medical
Research (ICMR) sanctioned the project “Hospital Based Cancer Registry and
Patterns of Care and Survival Studies on Cancer Breast, Cancer Cervix and Head
and Neck Cancers” at our Institute and thus we became a part of NCRP network.
The broad objectives of this study is to assess quality of hospital care and cancer
services in covered area and to describe length and quality of survival in relation to
anatomical site, clinical stage and aspects of types of treatment and to undertake
epidemiological research through short term case control studies.
The coming years will show to what measure we succeed in our efforts.

FOREWORD
I am pleased to write this foreword on this report of Hospital Based Cancer Registry
(HBCR) for the year 2015. The HBCR has over the years given an assessment of the
magnitude and patterns of cancer in the region being catered by the registry. It has
also contributed to the Population Based Cancer Registry of Ahmadabad Rural and
Ahmedabad Urban Agglomeration area. The HBCR have helped in patient
management of the Institute and its presence improved the upkeep of patient
medical records in a more scientific manner.
It is hoped that this report will serve as a guide to the treating oncologist, researchers
and health administrators to deal with the problem of cancer in different geographical
areas. It is the result of considerable effort put in by the registry staff and I appreciate
the dedicated work of all the concerned staff in bringing out this publication.

ABOUT GUJARAT CANCER & RESEARCH INSTITUTE...
Conceived during society’s efforts for health awareness and nurtured by the
Governor of Gujarat, this effort to ameliorate suffering and agony of patients with
cancer crystallised into formation of Gujarat Cancer Society on April 2, 1961.
Following the initial period of intensive spadework the first patient was examined on
October 2, 1965 and the M.P.Shah Cancer Hospital was commissioned on
December 14, 1966 by the late Prime Minister Mrs. Indira Gandhi. The M.P.Shah
Cancer Hospital grew, developed and then underwent metamorphosis into The
Gujarat Cancer & Research Institute on February 1, 1972.
The Gujarat Cancer & Research Institute (GCRI) is one of the largest comprehensive
cancer treatment centres in India and it is recognised as a Regional Cancer Institute
by the Government of India. Recently, it has been upgraded as State Cancer
Institute by Government of India. Patients referred from all over Gujarat and adjoining
areas of the States of Rajasthan, Madhya Pradesh and Maharashtra come to the
Institute. The Institute is also recognised by International Union against Cancer,
Geneva, as a Comprehensive Cancer Centre.
The Hospital at present has departments of Surgical Oncology, Medical Oncology

with BMT, Radiation Oncology, Gynaec Oncology, Paediatric Oncology, Neuro
Oncology, Orthopaedic Oncology, Reconstructive Surgery, Urology Surgery, Radio-
diagnosis, Anaesthesiology, Palliative Care Unit, Nuclear Medicine, Pathology

HOSPITAL BASED CANCER REGISTRY, AHMEDABAD
Hospital Based Cancer Registry (HBCR) of Gujarat Cancer & Research Institute
(M.P.Shah Cancer Hospital) is functioning since 1972. It is situated in Ahmedabad
City.
In view of the excellent facilities available for diagnosis and treatment of cancer, a
large number of cancer patients seek treatment from not only all over Gujarat State
but also from the neighbouring states like Rajasthan, M.P., Maharashtra and other
parts of the country also. About 30,000 cases (both cancer and non cancer) are
registered annually and around 3,80,000 outpatient visits are recorded each year.
The National Cancer Registry Programme (NCRP), National Centre for Disease
Informatics and Research (NCDIR), ICMR, Bengaluru sanctioned the project
“Hospital Based Cancer Registry- Patterns of Care and Survival Studies in Cancer
Cervix, Cancer Breast and Head & Neck Cancers” (HBCR-POCSS) from the year
2014 at Gujarat Cancer & Research Institute (GCRI) and hence HBCR, GCRI
became a part of NCRP, ICMR, Bengaluru.
The broad and overall objectives of a Hospital Based Cancer Registry vis-à-vis the
present HBCR-POCSS are outlined below:
1. Assess Patient Care;
2. Participate in Clinical Research to Evaluate Therapy;
3. Provide an idea of the patterns of cancer in the area;
4. Help plan hospital facilities;
5. Contribute to active follow-up of the cancer patient;
6. Describe length and quality of survival in relation to anatomical site, clinical stage
and aspects of types of treatment;
7. Contribute to the Population Based Cancer Registries (PBCRs) in the given area;
8. Undertake epidemiological research through short-term case control studies;
9. Show time trends in proportion of early to late stages at the time of diagnosis;
10. Help assess quality of hospital care and cancer services in covered area.
REGISTRATION SYSTEM AT GCRI
GCRI being a referral cancer centre, various medical institutions and private
practitioners mostly refer patients. An active registration process is followed. The
patients are allotted a case file with a registration number. Only the patients who are
diagnosed to have cancer are included in the Hospital Based Cancer Registry
(HBCR). The O.P.D. Registration number is entered in the HBCR files. No new
numbering system is adopted by the HBCR for the registration purposes. Single
Record and Hospital numbers are used for both outpatient and inpatient activities.
The registration system is computerised. All the new patients attending the hospital
OPD are interviewed by registry staff. Socio-demographic information, details of
medical reports given by the referring doctors etc. are entered. After six months of
registration, the case records of these patients are obtained for abstracting
information on medical items. Reported malignant neoplasms are classified and
coded as per the Manual of International Classification of Diseases, Injuries and
Causes of Death-tenth edition (ICD-10). International Classification of Diseases for
Oncology (ICD-O-III) has been also used for coding of microscopically verified
reports of pathology. Patient identifying and socio-demographic information are not
filled up in manual core-proforma provided by NCRP-NCDIR, ICMR as these
information is already there in Institute’s Hospital Information Management System
(HIMS) system. The diagnostic and therapeutic part including follow up are being
filled up in Core-Proforma. The information filled up in core proforma are entered in
online HBCR-POCSS software developed by NCRP-NCDIR. The NCRP-NCDIR
generates a series of consistency checks and send to us for modification/deletion.
After, removing or deleting the errors, the data is again sent to NCRP-NCDIR. After
performing all quality checks, the NCRP-NCDIR provides annexure tables. Based on
the annexure tables, the report is being prepared and published.
Integration of GCRI HIMS with NCDIR Software
As it is mandatory to enter the data in online HBCR-POCSS software developed by

NCRP-NCDIR, we were facing lot of duplication work while entering the information
like patient identifying information, diagnostic and therapeutic information after
abstracting the casefiles. From our HIMS, many of the information can be captured.
So, we raised our concern to integrate our HIMS with NCDIR HBCR-POCSS
software during the HBCR-POCSS workshop held at Bengaluru in February 2016.
The NCRP-NCDIR agreed and they have successfully integrated both the software.
Thus lots of duplication work, time and human resources are saved.

SUMMARY
During the year 2015, 22,133 (males: 13575; females: 8558) patients with cancer were
recorded in the Hospital Based Cancer Registry of the Gujarat Cancer and Research
Institute. The median age at diagnosis was 50 years in males and 49 years in females.
Children (0 – 14 years) constituted 3.26% and 68.26% of all cancers were in the
truncated age group (35 – 64 years). Male: female ratio was 1.59:1.
Out of total malignant cases, 16,719(75.54%) patients were from Gujarat, 3085(13.94%)
patients from Rajasthan, 1727(7.80%) patients from Madhya Pradesh, 132(0.60%) from
Maharashtra and 463 (2.09%) from other states. 7 cases were registered from other
countries.
Cancer of other parts of mouth (21.92%) was the leading site among males followed by
cancer of tongue (12.18%). Among females, cancer of breast (21.45%) was the leading
site followed by cancer of cervix (17.18%).
In children (0 -14 years) lymphoid leukemia (39.98% in boys & 35.06% in girls) was the
predominant cancer in both sexes. In the 15 – 34 year age group, mouth cancer
(24.51%) was the predominant cancer in males and cancer of breast (17.35%) in
females. In the 35-64 year age group, cancer of mouth (25.75%) was the predominant
cancer in males and cancer of breast (23.83%) in females. 65+ age group, the leading
cancer sites were bronchus & lung in males (15.53%) and breast in females (16.56%).
The proportion of tobacco related cancers relative to all cancers was 60.15% in males
and 20.94% in females.
Diagnosis by microscopic verification was available in 99.20% of males and 99.43% of

females.
A total of 11,519 (61.48%) of total cancer patients were given various cancer directed
treatment (excluding the patients previously treated). Radiotherapy and chemotherapy
were common modalities of treatment.

GEOGRAPHICAL DISTRIBUTION
Cancer patients from various districts of Gujarat state covered 16,719 (75.54%) of total
cancer cases. 5407 (24.43%) cancer patients from neighboring states like Rajasthan,
Madhya Pradesh, Maharashtra and other states like Uttar Pradesh were attended GCRI
during the year 2015. From abroad, 7 cases were registered at GCRI. Ahmedabad
district contributed to 18.99% of all cancer cases.

Following map shows geographical distribution of cancer cases in both sexes registered

AGE GROUP AND SEX DISTRIBUTION
Table 2 shows that the bulk of the cancer cases were from the age group 35-64 years
among both sexes (68.26%). The proportion of paediatric age group (0-14 years) and
geriatric age group (65+ years) was 3.61% and 19.29% respectively of total males while
2.70% and 14.68% respectively of total females. The male: female ratio in total cancer
cases was 1.58:1.

The average age of cancer patients was 50(SD=16.13) years among males and
49(SD=14.66) years among females. The highest number of cases was observed in the
age group 50-54 years in males (12.88%) as well as in females (15.05%). The relative
proportion of cancer cases in five-year age group is shown in Table 3 and also
represented in figure III(a) and III(b).

SITE GROUPS OF CANCER
Oral cavity & pharynx as a whole contributed 32.31% of total cancers among all site
groups. In males, oral cavity & pharynx as a whole was the commonest cancer site
group (43.54%) followed by respiratory organs (14.14%) and digestive organs (11.87%).
In females, most frequent cancers occurred in breast (21.45%) followed by cervix
(17.18%) and oral cavity & pharynx as a whole (14.50%).

TEN LEADING SITES OF CANCER IN MALE
Ten leading sites of cancer in males contributed 66.73% of all male cancers. Cancer of
Mouth (21.92%) ranked at the top followed by Tongue (12.18%). The other leading sites
are as shown in Table 5 and Figure IV.

TEN LEADING SITES OF CANCER IN FEMALE
In females, the first ten leading sites of cancer constituted 70.19% of all female cancers.
Cancer of breast (21.45%) was at the first place and cervix cancer (17.18%) was at the
second position. Together these two sites accounted for 38.63% of all female cancers.
Cancer of mouth ranked third followed by ovary as described in Table 6 and Figure V.

FIVE LEADING SITES IN PAEDIATRIC AGE GROUP (0-14 YEARS)
3.26% of total cancer cases were covered by paediatric age group. Leukemias and
lymphomas together constituted 62.04% and 54.98% of pediatric cancers in boys and
girls respectively. Lymphoid leukemia emerged as the commonest cancer among both
sexes and other rank order are as follows:

Adult age group comprised 10.97% of total cancer cases. The commonest cancer was
mouth (24.51%) in males and cancer of breast (17.35%) among females in the age
group of 15 – 34 years.

PAEDIATRIC CANCERS
721(3.26%) of the total cancer cases were contributed by paediatric cancers. The
number and relative proportion in five year age group categories in paediatric cancers
as boys was higher (31.84%) than girls (27.71%) in the age group 10-14 years as
mentioned in the table 15. The load of paediatric cancers in GCRI is shown in Figure X.

for 48.40% of total pediatric cancers followed by malignant lymphoma and Hodgkin’s

TOBACCO RELATED CANCERS
Tobacco related cancers (TRC) were 44.99% of total cancers. A total of 60.15% of all
cancers in men in comparison to 20.94% in women were directly attributable to the use
of tobacco.

METHOD OF DIAGNOSIS
21,975 (99.29%) cases were microscopically confirmed out of 22,133 new cancer
cases. Microscopic verification was found in 99.20% cases in male cancers and 99.43%
cases of female cancers. Number and proportion of cancers by different methods of
diagnosis are presented in the following table and diagram.

CANCER DIRECTED TREATMENT
There are many types of cancer treatment. Three main modalities of treatment of cancer
are (i) Surgery (ii) Radiotherapy and (iii) Chemotherapy. Some cancer patients will have
only one treatment whereas most of the patients have a combination of treatments, such
as surgery with chemotherapy and/or radiation therapy. Other treatment modalities for
cancer are immunotherapy, targeted therapy, or hormone therapy. Following table no.
19 shows the treatment status of cancer patients registered during the year 2015 at
GCRI.

History of previous cancer directed treatment

A history of having any cancer directed treatment (CDT) prior to registration at GCRI
was present in 3396 (15.34%) cancer cases in both the sexes. Among these 2756
(81.15%) undergone surgery, 403(11.87%) received radiotherapy, 981 (28.89%) were
given chemotherapy either alone or in combination. 25 (0.74%) received hormonal
therapy.

CANCER DIRECTED TREATMENT AT GCRI
At GCRI, 11,519 (61.48%) of cancer patients were given various cancer directed
treatment (excluding the patients previously treated). Among them 7,217 (61.10%) were
males and 4,302 (62.11%) were females. 7,218 (38.52%) cancer patients didn’t receive
any kind of cancer directed treatment after diagnosed as cancer.

Radiotherapy and chemotherapy were common modalities of treatment. 5,578 (67.48%)

of patients had undergone chemotherapy alone or in combination with other forms of
cancer directed treatment in both sexes. Radiotherapy was received by 4,372 (52.85%)
cancer patients either alone or in combination with other forms of CDT. Surgery was
performed in 2,935 (35.48%) patients either alone or in combination. 232 (2.80%)
cancer patients received Hormonal therapy either alone or in combination with other
forms of CDT.
