RETENTION POLICY

1.0 Purpose: To provide guideline, instructions & process of retention of medical records, data and
information of Medical Records.
2.0 Scope: Hospital wide
3.0 Responsibility: Medical Record Incharge
4.0 Authority: This Policy cannot be changed without the prior written approval from Chief Executive Officer.
5.0 Policy & Procedures:

Organization has standardized the formats of collection data from

with the serial number with Retention Period as per rules
 Out- Patient Record : Three (03) Years.

pendency of court case
Retention process provides confidentiality and security inside the

Destruction of medical records, data and information is done with the
prior approval from Management and record of the same is
5.6 recorded. Shredder is used for destroying medical records so that Incharge - MRD
confidentiality and security do not compromise. Before destroying
Public Notice is mandatory.

Retention period of In-Patient Records shall be as follows:


In Patient records - 10 years
Death / MLC in Patient Records - 6 years
Death register - 10 years
MLC registers - 10 years
Employees Medical Records - Till retirement as well as attached with ‘R’ number
Medical records with clinical research - 5 years
Medical records with adverse events - 5 years

OPD records - 3 years
Birth & Death records - 10 years
The records are destroyed by putting them in to a separate water tank and then the paper is recycled.
