TAKE NOTICE THAT application will be made on behalf of the abovementioned


applicant on the 12th day of MAY 2020 at 10 a.m. or so soon thereafter as counsel
may be heard for an order in the following terms:

1. That the applicant be allowed to practice as attorney of the above honourable


court.
2. That the registrar of the above honourable court be directed to enroll the
applicant’s name on the role of attorneys of the above honourable court.
3. That the alternative relief be granted as the above honourable court may
deem fit.

And that the affidavits of Mabel Mhize and Anna Seopa annexed hereto will be used
in support thereof.

FOUNDING AFFIDAVIT

I, the undersigned,

MABEL MKHIZE

do hereby make oath and say that-

1. I am the applicant in this application.

2. I am an adult female born on . I refer the above honourable court


to a certified copy of the relevant page of my identity document annexed
hereto marked “MM1”.

3. I reside at , Pretoria, which address falls within the jurisdiction


of the above Honourable Court.

4. The facts contained in this affidavit are, unless the context otherwise
indicates, within my personal knowledge and are, to the best of my knowledge
and belief, both true and correct.

5. I respectfully submit that I am duly qualified to be admitted and enrolled as an


attorney having met the requirements of sections 15 and 16 of the Attorneys
Act 53 of 1979, as emended (the “Act”) by virtue of the following facts:

5.8.1 my estate has not at any time been sequestrated whether


provisionally or finally, nor to the best of my knowledge are
any sequestration proceedings against my estate pending;

5.8.2 to the best of my knowledge there are no civil, criminal or


administrative proceedings to be instituted against me;

5.8.3 I had not had any civil judgments granted against me nor
have I been convicted of any criminal offence nor incurred
any administrative penalty;

5.8.4 to the best of my knowledge there are no disciplinary


proceedings by the Law Society of the Northern Provinces
(or its predecessor) pending or about to be instituted against
me, nor have any such proceedings ever been instituted
against me in the past;

5.8.5 I have never previously been admitted a an attorney or


advocate of any division of the High Court of South Africa,
nor have I previously made any application for such
admission. I have not been struck from the Roll of Attorneys
or from the Roll of Advocates or suspended from practice as
an attorney or advocate by this honourable court;

5.8.6 I have never been found guilty of any misconduct or


disciplinary proceedings while in a previous profession or
employment or in my current employment or at University,
nor was any profession relinquished or employment
terminated while misconduct proceedings were pending
against me, nor any disciplinary actions currently pending
against me.
WHEREFORE I pray that this honourable court grant an order in terms of the notice
of motion to which this affidavit is annexed.

The deponent has acknowledged that she knows and understands the contents of
this affidavit, which was signed and affirmed before me at JOHANNESBURG on this

2. In the alternative that the applicant be granted an interim interdict.

deem fit.

thereof.

TAKE NOTICE FURTHER that the applicant has appointed Josephine Simpson

will accept notice and service of all process in these proceedings.

TAKE NOTICE FURTHER that if you intend opposing this application you are
required to (a) notify the applicant’s attorney in writing on or before the second day of
May 2010 (b) and within fourteen days of the service of this notice upon you to file
your anserine affidavits, if any, and further that you are required to appoint in such
notification an address referred to in rule 6(5)(b) at which you will accept notice and
service of all documents in these proceedings.

If no such notice of intention to oppose be given, the application will be made on the
26th day of August 2014 at 10:00.
